This paper will introduce the process and required materials for the replacement of the lost nurse's practice certificate.

Required materials

1. Certificate of loss reporting issued by the registered unit (official seal of the unit is required); 2. Individuals fabricate applications; 1. Copy of the applicant's ID card (double-sided copy on the same side of a piece of A4 paper) (copy 1 copy); 2. Bare-headed ornaments on the front, two-inch recent photo on white background (1 piece).

Treatment process

1. The applicant shall bring the above materials to the local health talent exchange center for review and be collected by the medical education network; 2, after the audit report to the provincial health department nurse registration office for examination and confirmation; 3. The applicant will come to our center to get a replacement card with his ID card within 30 working days after submitting the materials. (Original ID cards of both parties are required for collection).
