Processing time

The physical examination time is 8: 00 am-10: 30 am every Monday, Tuesday, Thursday and Friday, with a daily limit of 20 people. Please make an appointment by phone one day in advance and come in strict accordance with the appointment time. If you don't make an appointment or come at the appointed time, it will not be handled.

Required materials

1. The holder of the household registration certificate of Beizhai Street must provide a copy of his ID card and two one-inch photos.

2. If you are not registered in Beizhai Street, you need to provide a copy of the business license of Laoshan District, and affix the official seal, a copy of your ID card and two one-inch photos on the front and back of the copy.

Where to obtain certificates and evidence

There are 2 consulting rooms on the first floor of the outpatient building of Beizhai Health Center. The time for obtaining evidence is in the morning after 1 week (no evidence will be obtained on Wednesday and weekend). Please remember the evidence number.
