A period of work ended inadvertently. Looking back on this period, our working ability and experience have grown. It's time to write a work summary seriously. In order to make it easier and more convenient for you to write a work summary, the following is a summary of hospital prescription comments I collected for your reference only. Let's have a look.

Summary of Hospital Prescription Review: 1 20xx Article 44 of the Prescription Management Measures of the Ministry of Health stipulates that medical institutions should establish a prescription review system, fill out a prescription evaluation form, implement prescription dynamic monitoring and extraordinary early warning, register and notify unreasonable prescriptions, and timely intervene in irrational drug use. In 20xx, the Ministry of Health issued the Management Standard for Hospital Prescription Review (Trial). According to the above regulations, our hospital insists on reviewing prescriptions once a month from 20xx. The related work report is as follows:

1, prescription review work

Prescription comments in our hospital are jointly organized and implemented by the Medical Department and the Pharmacy Department under the leadership of the Hospital Pharmacy Management and Drug Treatment Committee and the Medical Quality Management Committee. According to the actual situation of our hospital, 300 outpatient prescriptions and 50 inpatient medical records were randomly selected every month, and experts in clinical, pharmacy and administrative management were organized to conduct outpatient consultation according to Prescription Management Measures, Management Measures for Clinical Application of Antibacterials, Special Remediation Plan for Clinical Application of Antibacterials of the Ministry of Health, Guiding Principles for Clinical Application of Antibacterials, Instructions for Clinical Use of China Pharmacopoeia and Newly Edited Instructions for Pharmacology and Drugs. The content of prescription review includes the standardization of prescription writing and the suitability of clinical application of drugs, including indications, drug selection, route of administration, usage and dosage, drug interaction, incompatibility and so on. When existing or potential problems are found, intervention and improvement measures are formulated and implemented to promote clinical rational drug use. The evaluation indexes include the average variety of drugs used in each visit, the dosage of antibacterial drugs used in each visit, the injection dosage used in each visit, the proportion of generic names used, the proportion of essential drugs in prescription drugs, and whether there are unreasonable drugs. Focus on the analysis of inappropriate prescriptions and abnormal prescriptions found in the review, and report the review results in the whole hospital.

2. Publicity and education on rational drug use

Our hospital has carried out a lot of publicity and education work, including: ① convening a special work meeting of the leading group for the special rectification of antibacterial drugs and a mobilization meeting for all staff of the special rectification activities for clinical use of antibacterial drugs. Hospital leaders and department directors signed the Responsibility Letter for Clinical Rational Use of Antibacterials. ② Use drug news and hospital display screen to carry out special rectification of antibacterial drugs and publicize rational drug use. ③ In view of the problems existing in the use of antibacterial drugs, traditional Chinese medicine injections, proton pump inhibitors and vitamins in our hospital, we invited experts and professors from superior hospitals to give lectures to solve the difficult problems in drug use. ④ Pharmacists use the drug consultation window to publicize the basic knowledge of antibacterial drug use and the harm of abuse of antibacterial drugs to patients and their families.

3, formulate relevant systems, and pay attention to implementation.

While carrying out publicity and education, our hospital has also done the following work:

(1) formulated a special rectification activity plan for antibacterial drugs, set up a leading group and a special work class, and adopted the principle of giving priority to education, paying equal attention to accountability and economic punishment, focusing on the outstanding problems and key links in the clinical application of antibacterial drugs.

② The variety of antibacterial drugs shall be controlled at no more than 35, and the regulation of "one product and two regulations" shall be strictly implemented.

(3) Formulated the relevant system to control the proportion of drugs, stipulated that the Hospital Pharmacy Management and Pharmacotherapeutics Committee was responsible for the supervision and management of rational drug use in the hospital, and set up an expert supervision team for rational drug use in the hospital. The director of each clinical department is the first responsible person for rational drug use in the undergraduate department, who is specifically responsible for supervising and managing the rational drug use and prescription of undergraduate courses and correcting the problems existing in the clinical drug use of undergraduate courses in time.

(4) Statistics of the top 3 drugs used, suspended 1 month.

⑤ Implement the publicity system of drug proportion and prescription comment, inform the monthly drug proportion of doctors in front of the department 10 and the whole hospital, admonish the doctors who have irrational drug use, and give financial punishment and suspend prescription qualification if the circumstances are serious; Give financial penalties to departments and individuals whose drug proportion exceeds the standard. According to the results of prescription review, our hospital admonished many doctors for irrational drug use and suspended prescriptions 1 month.

Through the implementation of prescription review, the clinical medication in our hospital tends to be reasonable, and the irregular medication and prescriptions are obviously reduced. The management index of clinical application of antibacterial drugs has basically reached the regulations of the higher health administrative department, and the proportion of drug use has dropped from 47% before prescription review to below 40%. It is believed that the application of rational drug use management software will greatly facilitate the prescription review work, and our hospital will pay more attention to it and carry it out in a down-to-earth manner.

Summary of Hospital Prescription Reviews 2 In order to standardize prescription reviews in our hospital, improve prescription quality, promote rational drug use and ensure medical safety, according to the Measures for Rational Use of Antibacterials and Prescription Management, the Prescription Review Management System of Mongolian Hospital in Zhungeer Banner and other related systems, the prescription review working group of our hospital randomly selected prescriptions in xxxx, aiming at standardizing prescription management, standardizing clinical rational use of antibacterial drugs and improving the clinical drug treatment level of our hospital.

Comments: Before the 5th day of each month, the prescriptions for 7 days of last month were randomly selected for comments, and 4,846 prescriptions were randomly selected throughout the year, with an average of 403.8 prescriptions randomly selected every month. The results of spot check in xxxx year: rational prescriptions accounted for 77.6%, and irrational prescriptions were mainly irregular prescriptions; On average, 2.56 kinds of drugs were used in each prescription, and the consumption amount was 47.8 yuan. The utilization rate of essential drugs was 57.4%, that of generic names was 98.7%, that of injections was 22.9%, and that of antibacterial drugs was 27.8%.

Analysis of nonstandard prescriptions: nonstandard prescriptions include fees, subjects, addresses and incomplete diagnosis; There is no doctor's signature or reason to modify the prescription or use drugs for over-course; Large prescriptions are not signed by patients; Omission of unit and address; In addition, the application of antibacterial drugs in individual prescriptions is not standardized.

Analysis of irrational drug use: there are repeated drug use in irrational drug use prescriptions, and the drug diagnosis and prescription are not symptomatic. Individual prescriptions have errors in drug dosage form, specification, usage and dosage, and occasionally excessive dosage and concentration are given intravenously.

Application of Antibacterials: On the whole, the proportion of prescriptions of antibacterial drugs in the total prescriptions is declining, and the graded use of antibacterial drugs is becoming more and more standardized. However, there are still cases where antibacterial drugs are not selected according to the characteristics of antibacterial drugs and the types of pathogenic bacteria, which need further improvement.

Put forward suggestions for improvement in view of the existing problems;

1. I hope that doctors from all disciplines will strengthen cooperation with pharmacies, make clinical departments pay more attention to the correct writing of prescriptions, strengthen supervision over irregular writing of prescriptions, and take corresponding measures for unqualified parts according to the prescription review system in our hospital;

2. It is suggested that the Information Department establish procedures to prevent incomplete prescriptions from passing, and eliminate errors in incomplete items such as age, usage and dosage from the system;

3. I hope that clinicians will actively cooperate with the implementation of the hospital system, improve the quality of prescriptions in our hospital, promote rational drug use, ensure medical safety, and reduce the occurrence of unqualified prescriptions.

Summary of hospital prescription review work 3 i. Prescription review table shows that

100 outpatient prescriptions were randomly selected in October, with an average of 2.2 kinds of drugs used in each prescription. The utilization rate of injections was 15.0%, the utilization rate of antibacterial drugs was 32.0% (higher than the target of 20% set by the Ministry of Health), and the utilization rate of generic drugs was 100%, with an average of each prescription.

Second, the main problems

1, the outpatient prescription is more than 7 days, and the reason is not indicated;

2, the indications are not suitable;

3, no indications for medication.

Some unreasonable prescriptions are as follows:

Third, analysis

The qualified rate of prescriptions this month is higher than that of last month, and the problems such as inadequate pharmacists' signatures and improper drug usage and dosage have been obviously improved. However, the incompatibility of indications and the existence of prescriptions without indications are still the main problems to be solved. In addition, the utilization rate of antibacterial drugs is much higher than the 20% stipulated by the Ministry of Health. It is found that there are more oral antibacterial drugs and more seasonal diseases such as bronchitis this month, which are the main reasons for the increase in the utilization rate of antibacterial drugs.

Fourth, improvement measures:

1. Unreasonable prescriptions this month are mainly drugs with no indications and prescriptions with inappropriate indications. Many patients hold a medical card or a patient visits multiple departments, and doctors prescribe drugs directly without modifying the diagnosis. In order to avoid such prescriptions, doctors should strictly implement the relevant provisions in the Prescription Management Measures, and the clinical diagnosis should be clearly and completely filled in and consistent with the medical records; Improve the information system and put an end to such prescriptions.

2. The network center and other relevant departments constantly improve the information system, support the electronic prescription review and special prescription review of antibacterial drugs, avoid subjective errors, and make the data more universal and reasonable.
