The general procedures for applying for reasonable accommodation shall include:
(1) Disabled candidates who sign up for the college entrance examination and apply for reasonable convenience shall submit a formal written application in accordance with the time, place and method stipulated by the provincial education examination institutions. The contents of the application should include my basic information, disability, the reasonable convenience of the application, and the items that need to be carried with me. , and provide my second generation and above "People's Republic of China (PRC) (People's Republic of China (PRC)) Disabled Card" and its original and photocopy (scanned copy) and valid identity documents stipulated by provincial education examination institutions.
(II) The educational examination institutions are responsible for accepting and reviewing the formal applications submitted by disabled candidates who take the examinations locally, leading the organization of expert groups composed of professionals from relevant educational examination institutions, the Disabled Persons' Federation, health and other relevant departments to confirm the identity and disability of disabled candidates on the spot, and make a comprehensive evaluation according to the degree of disability, daily study, reasonable and convenient application and examination organization conditions of disabled candidates, and form a written evaluation report.
(3) The provincial education examination institutions shall, according to the evaluation opinions of the expert group, form a Notice on the results of the national unified examination for the enrollment of disabled candidates in ordinary colleges and universities (hereinafter referred to as the Notice), and deliver the Notice to disabled candidates within the prescribed time limit, which shall be confirmed and signed by disabled candidates or legal guardians. The contents of the notice shall include the basic information of the disabled candidates, the detailed contents of the decision of the examination institution, the reasons and basis for the decision, and the relief channels.
