First, the scope and object of identification
Scope of accreditation: persons who apply for accreditation as practice instructors in senior middle schools, secondary vocational schools and secondary vocational schools.
Appraisal object: 1. China citizens who have not reached the national statutory retirement age, and whose household registration, residence (local residence permit is required and within the validity period) and school location (full-time undergraduate graduates and graduate students only) are in Ya 'an.
2. Active servicemen and armed police stationed in Asia.
3. Hong Kong, Macao and Taiwan residents who hold residence permits for Hong Kong, Macao and Taiwan residents can apply for teacher qualifications at their place of residence; Hong Kong and Macao residents who hold Hong Kong and Macao residents' passes to and from the mainland and residents of Taiwan Province Province who hold mainland travel permit for taiwan residents valid for five years can apply for teacher qualification in the place where they take the qualification examination for primary and secondary school teachers.

Four, the applicant on-site confirmation of the materials to be submitted

Special note: In the process of material review, applicants must submit the China Higher Education Qualification Report (Xuexin.com Online Application) for academic qualifications that cannot be directly compared and verified by the national information system (except secondary vocational school qualifications), otherwise they will not be treated as unqualified academic qualifications. The 2022 graduate students of education and normal students at public expense provide teachers' professional ability certificates (which can be verified by the system, and are not required to submit), and other personnel provide the original certificate of primary and secondary school teachers' qualification examination issued by the Examination Center of the Ministry of Education (the Teacher Qualification Examination Center of the Ministry of Education), which is printed by the applicant on the National Primary and Secondary School Teachers' Qualification Examination Network (ntce.neea.edu.cn) (which can be verified by the system, and is not required to submit).
9. Those who apply for the qualification of internship instructor in secondary vocational schools shall, in addition to providing the above information, also provide the professional title certificate equivalent to the professional technical position of assistant engineer or above or the technical level qualification certificate of intermediate workers or above.
Verb (abbreviation for verb) physical examination
If it is necessary to submit the physical examination form for on-site confirmation, the applicant should arrange time to take the second-generation ID card and 1 recent bareheaded front 1 inch color white background photo to the designated medical examination hospital to receive the physical examination form for physical examination before on-site confirmation.
The physical examination hospital applying for the qualification of internship instructor in senior middle schools, secondary vocational schools and secondary vocational schools is: Affiliated Hospital of Ya 'an Vocational and Technical College (Yucai Road, Yucheng District).
Other matters of intransitive verbs
1. Applicants are requested to make online declaration and on-site confirmation according to the specified time, place and requirements. If the application is not completed within the specified time due to reasons such as missing the application time, choosing the wrong accreditation institution or on-site confirmation point, incorrect application information or incomplete submission materials, the accreditation institution will no longer accept the application, and the responsibility will be borne by the applicant himself. 2. Applicants must upload photos as required when registering online, and the photos are consistent with the photos pasted on the Physical Examination Form for Teachers' Qualification Candidates in Sichuan Province and the photos of teachers' qualification certificates.

4. Applicants should truthfully submit relevant materials, and those who deliberately resort to deceit to cheat teachers' qualifications will be punished according to relevant state regulations.
