In order to standardize the replacement, renewal and information correction of teacher qualification certificates in Xi and protect the legitimate rights and interests of holders of teacher qualification certificates, the Education Bureau now issues the following provisions: Measures for the Implementation of Regulations on Teacher Qualification, Regulations on the Management of Teacher Qualification Certificates, Interim Measures for the Regular Registration of Teachers' Qualifications in Primary and Secondary Schools, Notice of the Guidance Center of the Ministry of Education on standardizing the renewal of teacher qualification certificates, and Notice of the Shaanxi Provincial Department of Education on standardizing the renewal of teacher qualification certificates.
I. Scope of acceptance
Teacher qualification certificate issued by Xi Education Bureau (please pay attention to the announcement issued by the county education bureau for the renewal, renewal and information correction of teacher qualification certificate issued by the county education administrative department).
Second, the replacement, renewal and information correction of teacher qualification certificate.
(1) The teacher qualification certificate is reissued as follows: the teacher qualification certificate is lost;
(2) the renewal of teacher qualification certificate: the holder's teacher qualification certificate is seriously damaged, which affects the normal use;
(3) The correction of teacher qualification certificate information is that the teacher qualification certificate information is not standardized or wrong, including the holder's personal information and certificate number.
Three, teacher qualification certificate replacement, renewal, information correction procedures
(a) the applicant to the issuing authority (Xi Education Bureau) to apply and submit the corresponding materials;
(two) the license issuing agency to review the materials submitted by the applicant;
(three) after the certification body has passed the examination, it shall enter the information, make the certificate and issue the certificate.
Four, the teacher qualification certificate replacement, replacement, information correction materials to be submitted.
(a) to apply for a replacement or renewal of teacher qualification certificate, the following materials shall be submitted to the issuing agency:
1. application form for teacher qualification certificate renewal (applicable to primary and secondary schools) (see annex 1) in duplicate (this form is also applicable to kindergarten and secondary vocational school teacher qualification certificates);
2. A copy of the teacher qualification application form (1996 certificate) or the teacher qualification application form (certificate 2001-2021), which shall be marked as "consistent with the original" by the file storage institution and stamped with the official seal of the file storage institution;
3. Original and photocopy of my valid resident ID card or household registration book (1 copy);
4. 1 Recent color bareheaded photo with white background (name, qualification type and subject clearly written on the back), which is the same as the photo in the application form for teacher qualification certificate replacement;
5. In addition to the above-mentioned 1-4 materials, the power of attorney issued by the client (holder) (see Annex 3) and the original and photocopy of the client's ID card (the front and back of the ID card are copied on the same A4 paper) shall be provided.
Note: The original teacher qualification certificate must be provided when the teacher qualification certificate is renewed.
(two) the following materials shall be submitted for the correction of teacher qualification certificate information:
1. The information correction and filing form of teacher qualification certificate is in duplicate (see Annex 2);
2. A copy of the teacher qualification application form (1996 certificate) or the teacher qualification application form (certificate 2001-2021), which shall be marked as "consistent with the original" by the file storage institution and stamped with the official seal of the file storage institution;
3. Original and photocopy of my valid resident ID card or household registration book (1 copy);
4. Original teacher qualification certificate;
5. To correct the ID number on the certificate, it is necessary to provide the original and 1 copy of the ID number change certificate issued by the public security department (check the original and keep the copy);
6. In case of entrusting others to handle the matter, in addition to the above-mentioned materials in point 1-5, the power of attorney issued by the entrusting party (see Annex 3) and the original and photocopy of the entrusted party's ID card (the front and back of the ID card are copied on the same A4 paper) shall be provided.

(4) Applicants need to check their health code, travel code, negative proof of 72-hour nucleic acid test, accept body temperature test, wear masks and do personal protection.
