(2) Steps for filing an application
1, fill in the application form.
To apply to this organ, it shall fill in the Application Form for Shanxi Provincial Government Information Disclosure (see the attached table, hereinafter referred to as the Application Form). A copy of the application form is valid and can be obtained from the accepting institution or downloaded from its website. In order to improve the efficiency of processing applications, the applicant should describe the required information as clearly as possible; If possible, please provide the title, release time, document number or other tips to help the organization determine the information carrier.
Step 2 submit an application form
Applicants can fill in a complete application form by letter, email or in person. (3) Application processing
After receiving the application, this organ will conduct a formal review on whether the elements of the application are complete. If the elements are incomplete, it will return the application and ask the applicant to supplement it.
If the information applied for belongs to the information that has been voluntarily disclosed by this organ, this organ shall suspend the application procedure and inform the applicant of the ways and means to obtain the information.
The agency processes applications according to the order in which they are received. If several independent requests are made in one application at the same time, the agency will give a unified reply after all the requests are processed. In view of the fact that different requests may have different answers, in order to improve the processing efficiency, it is suggested that applicants apply separately for different requests.
(4) Time limit for handling
If an organ receives an application for government information disclosure and can reply on the spot, it shall reply on the spot; If you can't reply on the spot, you should reply within 15 working days from the date of receiving the application; If it is necessary to extend the reply period, it shall be approved by the person in charge of the government information disclosure work agency, and inform the applicant that the longest extension of the reply period shall not exceed 15 working days.

Third, the way of supervision.
