Please note the following points:

1. Forklift driver's license belongs to special vehicle driver's license. You should apply to the forklift training base designated by the national and municipal quality supervision bureau. When applying, you need to bring the original and photocopy of your ID card and two one-inch photos.

2. According to the regulations, the forklift driver's license issued before 20 1 1 shall be verified every two years, and the forklift driver's license issued after 20 1 1 shall be verified every four years, and the annual review time shall be four days. Moreover, those who received the certificate for the first time and those who have been examined for the year are all together. They study three days ago and have an exam on the last day. After passing the exam, the annual review will be conducted every year. The annual inspection of forklift license is handled in the Quality Supervision Bureau, which is not restricted by the region.

The forklift truck annual inspection shall submit an annual inspection application to the quality supervision department, and provide the following materials:

1. Fill in the Application Form for Obtaining Evidence for Special Equipment Operators;

2. Two photos;

3. Copy of ID card;

4. Original operation certificate; 5. I personally participated in the training completion form.
