The forklift truck annual inspection shall be submitted to the quality supervision department, and the following materials shall be provided:

1. Fill in the Application Form for Obtaining Evidence for Special Equipment Operators;

2. Two photos;

3. Copy of ID card;

4. Original operation certificate;

5. I personally participated in the training completion form.

Forklift driver's license belongs to special vehicle driver's license, which is issued by the State Administration of Work Safety and has the People's Republic of China (PRC) Special Operation Certificate, and needs to be handled by the local quality and technical supervision department. According to the regulations, the forklift driver's license must be reviewed every two years, and the holder should apply for a review 1 month before the expiration of the certificate. Fails to review or review unqualified, shall be cancelled by the competent department.
