(2) Failing to set up a safety production organization or provide safety production personnel;

(three) the mine manager does not have professional knowledge of safety;

(four) special operations personnel have not obtained the qualification certificate to work at their posts;

(5) Failing to conduct pre-job safety education and training for employees;

(6) Failing to distribute labor protection articles required for ensuring safety in production to employees.
