Items marked with * are required, and then sent to authorized personnel to register and maintain the organization, which will be reviewed by professional teachers. The maintenance fee of CISP qualification certificate is 500 yuan/year, and the maintenance certificate is produced and issued after approval by the center.

Preparation of application materials for CISP qualification certificate maintenance:

After the approval, please prepare relevant materials according to the contents of the email reply and save the materials in the electronic scanning version.

(1) color scanned copy of the application form: printed on both sides; Note that the "current work unit" needs to be stamped with the official seal of the unit (the name of the unit must be consistent with the actual official seal), and the fifth part of the personal statement needs to be signed by hand.
