1. Two, occupational health records include:
1, occupational health record card of enterprises and institutions;
2. Schematic diagram of the distribution of occupational hazard factors in each production unit of the factory and the distribution of key occupational hazard factors in each production unit of the factory;
3. Process flow chart of each production unit of the factory (as shown by arrow);
4. List of personnel exposed to occupational hazards in each production unit;
5, the production unit contact with occupational hazard factors of workers registration card;
6. Record cards of occupational hazards and occupational poisoning;
7, occupational hazard factors test results summary data;
8, occupational disease risk factors classification management table;
9, occupational poisoning accident report and processing records, etc. ;
10, list of laws, regulations, norms, standards, documents, regulatory documents and related texts on occupational disease prevention and control;
1 1, occupational health management policies, plans, objectives, schemes, procedures, instructions and management systems;
12. Management organization, functions and personnel division of occupational health professional (part-time) posts;
13, occupational hazard management files of construction projects;
14, technical archives of occupational disease prevention measures;
15, archives of occupational disease protection facilities and protective articles;
16, summary data of occupational health training and education;
17, emergency rescue plan for occupational accidents and related data of drills.
Three, employee personal health records include:
1, employee's occupational history, past history and occupational hazard exposure history;
2. Monitoring results of occupational hazard factors in the workplace;
3, occupational health examination results, list of occupational contraindications and treatment;
4. Personal health information of employees such as occupational disease diagnosis certificate and occupational disease case registration form;
5. Power of attorney or contract for occupational health monitoring;
6, occupational health examination posts and personnel list;
7, occupational disease patients treatment, resettlement summary data.
Four, "two files" data according to the requirements of file management to establish a directory, unified numbering, special volume registration; Timely filing in three periods: permanent, long-term and short-term.
Five, "two files" data should be legible, clear charts, accurate and reliable text, and manage the "two files".
6. Adjust and supplement the "two files" at any time and on a regular basis according to the changes of the company's personnel, and adjust the system of each table card once a year before 10.
Seven, all kinds of information in the "two files" file shall be reviewed once every three years as required; Daily occupational health work must record the test results, health examination results and occupational disease management at any time for analysis.
Eight, employees leave the unit, have the right to ask for personal health files and reply files, files should be truthfully provided free of charge, and sign the personal copy provided.
Nine, the occupational disease diagnosis and identification units need to provide information about the "two files", the archives department should provide it truthfully.
10. The archives room should carefully check the quality of occupational health files handed over by various departments. The filing documents shall be filled in the handover catalogue, signed by both parties, numbered and registered in time, and put into storage.
Eleven, archivists should register the collection, removal, destruction, management, borrowing and utilization of archives, archives personnel must go through the handover procedures.
Twelve, occupational health archives warehouse should be solid and safe, do a good job of anti-theft, fire prevention, insect prevention, rat prevention, high temperature prevention, moisture-proof, ventilation, and emergency measures. Occupational health files should be managed by designated personnel, checked and counted regularly, and repaired and copied in time if the files are damaged or deteriorated.
Thirteen, the use of occupational health records personnel should take good care of the files, occupational health records are strictly prohibited, and it is strictly forbidden to open, alter, deface, lend and reprint occupational health records without permission.
Fourteen The right to interpret this regulation belongs to the occupational health and safety department of the company. Evaluate and revise its compliance according to the company's procedures every year.
Fifteen, this system shall come into force as of the date of promulgation. Other provisions on the management of occupational archives shall be implemented in accordance with the current national laws and regulations, occupational health standards and the company's occupational health management system.
Occupational health monitoring file management system
In order to protect the health of workers and safeguard the rights and interests of the company, according to the provisions of the Law of People's Republic of China (PRC) on the Prevention and Control of Occupational Diseases, the employer establishes occupational health monitoring files for workers and keeps them properly, and formulates this system.
1. The HSE Department is responsible for establishing occupational health monitoring files for each employee and keeping them properly.
Occupational health monitoring files include:
(1) Employee's occupational history, previous occupational history and occupational hazard exposure history;
(2) Monitoring results of occupational hazard factors in corresponding workplaces;
(three) the results of occupational health examination and its handling;
(4) Health information such as occupational disease diagnosis and treatment.
2. Archivists must safeguard workers' right to occupational health privacy and confidentiality. Relevant health supervision and inspection personnel, workers or their close relatives or their entrusted agents have the right to consult and copy the occupational health monitoring files of workers, and other personnel may not consult the occupational health monitoring files without permission.
3. When the laborer leaves the unit, I have the right to ask for a copy of the health monitoring file, and the file manager shall provide it truthfully and free of charge, and sign the provided copy.
4. The occupational health monitoring files of employees who have left the company should be sealed within three months after leaving the company and kept for more than 10 years for reference by higher authorities.
5. The file management personnel shall properly keep the occupational health monitoring files to prevent moth-eaten, mildew-proof, loss-proof and loss, and ensure the safety of the files.
6. All files should be stored in special counters, locked, cleaned regularly, ventilated and moistureproof.
7. All files shall not be consulted and copied at will, and shall not be placed in public places.
8. Others shall be implemented in accordance with the company's document management system.
9. This system shall be implemented as of the date of issuance.
