3. Medical examination and training fees:
① Employees in food production and management, drinking water and general public places: 1 10 yuan (medical examination fee) +20 yuan (training fee).
② Employees in special public places: 126 yuan (medical examination fee) +20 yuan (training fee).
Note: 1. Public places industry training once every two years. Those who have been trained in the previous year are exempted from training with the certificate of health knowledge training.
3. Special public places mainly include leisure, foot bath, sauna, bathroom and ballroom (KTV, karaoke hall, etc. ), entertainment and leisure places, hotel housekeeping, beauty salons (except simple haircuts) and other industries.

Those who have completed the physical examination and training and passed all the requirements can get the health certificate and health knowledge training certificate (2 certificates are 1) after four working days (those who have a physical examination on Monday can get the certificate on Friday afternoon; If you have a physical examination on Wednesday, you can get the certificate next Tuesday afternoon.

① One-inch bareheaded photo of myself (please write my name on the back of the photo) and about10g of my feces on the day of physical examination.
Pay attention to rest the day before physical examination, avoid overeating, avoid excessive fat diet, don't stay up late, and be too tired. Pregnant women should not have X-rays.
③ Blood was drawn on an empty stomach on the morning of physical examination. Please wear loose clothes during the physical examination. After blood drawing, please use sterile cotton swab to press the local area for 3 to 5 minutes, but don't rub it. Compression time is too short or rubbing is easy to produce hematoma.
7. Other matters:
① Those who fail the physical examination or training cannot receive the health certificate and health knowledge training certificate (2 certificates are 1). If I request a re-examination, I can come to our center to re-examine the unqualified items with my ID card during the normal medical examination time (every Monday and Wednesday morning).
② If the health certificate and health knowledge training certificate (2 certificates are 1) are lost or damaged and can't be used, you can submit them to our center free of charge with the unified bill of physical examination or the certificate of loss of health certificate issued by your unit at the time of physical examination, and with your ID card and one-inch bareheaded photo.
③ Self-employed persons holding employment assistance certificates (except entertainment, sauna, massage, Internet cafes, oxygen bars and other industries restricted by the state) can be exempted from paying the three-year health examination fee. When handling, you must go through the filing formalities at the office on the second floor of the front yard with your original employment assistance certificate, ID card and business license.
(4) Employees with disability certificates can reduce the medical examination fee 20 yuan.
