(1) Lack or defect of protection, safety and signal devices: ① No protection: no protective cover, no safety device, no alarm device, no safety sign, no fence or fence damage, (electrical) ungrounded, poor insulation, no silencing system for local ventilator, loud noise, working in dangerous buildings, no car stopper or fence to prevent "sports cars". ② Improper protection: the protective cover is not in place, the protective device is not properly adjusted, the tunnel excavation and tunnel excavation support are improper, the safe position of logging and skidding operations is not enough, the car stop net is defective, and the live part of the electrical device is exposed.
(2) Defects of equipment, facilities, tools and accessories: improper design, structure not meeting the safety requirements, access door blocking the line of sight, defective braking device, insufficient safety spacing, defective parking net, sharp burrs and burrs on workpieces, and sharp chamfers on facilities.
(3) Insufficient strength: the mechanical strength and insulation strength are insufficient, and the ropes for lifting heavy objects do not meet the safety requirements.
(4) The equipment operates under abnormal conditions: sick operation and overload operation.
(5) Poor maintenance and adjustment: equipment in disrepair, uneven ground, improper maintenance and equipment failure.
(6) Personal protective equipment-protective clothing, gloves, goggles and masks, respiratory protective equipment, hearing protective equipment, safety belts, helmets, safety shoes, etc. Whether it is missing or defective: ① No personal protective equipment or equipment. ② The protective articles and appliances used do not meet the safety requirements.
(7) The production (construction) site environment is poor: ① The lighting is poor, the brightness is insufficient, the work site is filled with smoke and dust, things can't be seen clearly, and the light is too weak. ② Poor ventilation: there is no ventilation, the ventilation system is inefficient, the airflow is short-circuited, the blasting operation is carried out when the power is cut off and the wind stops, the gas emission does not reach the safe concentration, and the blasting operation is carried out when the gas concentration exceeds the limit. ③ The workplace is narrow and messy, and tools, products and materials are not safely stacked.
(8) The traffic line configuration is unsafe, the operation program design or configuration is unsafe, the ground is slippery, there is oil or other liquid on the ground, the ground is covered with ice and snow, or there are other wet objects on the ground.
