To your city (county) Quality Supervision Administration and apply for a special equipment operator's certificate according to the Special Equipment Safety Law and the Regulations on Equipment Safety Supervision, the personnel shall meet the following conditions:

1, age 18 and above 60; Unless there are special requirements in the safety technical specifications.

2. Be in good health and meet the special requirements of the type of operation applied for;

3. Have an education level suitable for the application type;

4. Have corresponding safety technical knowledge and skills;

5. Meet other requirements specified in safety technical specifications;

6, through the provisions of the professional and technical theory and practical operation examination, the results are qualified.

Application materials provided:

(The application materials need to be stamped with the official seal, and the copies of relevant personnel's identity documents and personal qualification certificates need to be signed by myself. )

1. Operator Application Form (standard format of Quality Supervision Bureau, including training and work experience certificate; 1 serving);

2. Copy of the applicant's ID card (1 copy);

3. Copy of the applicant's graduation certificate or academic certificate (1 copy);

4. Physical health certificate including vision, color blindness, etc. (1 serving);

5. Notice of examination results issued by the examination institution (1 copy, which is unnecessary if the examination institution is entrusted to apply).

The quality supervision department shall, within 5 working days from the date of receipt of the application materials, make a decision on acceptance or rejection, or inform all the contents that need to be supplemented at one time. If the application materials need to be supplemented, the application shall be terminated after the applicant returns the application materials. After making corrections to the application materials, the applicant may reapply.

The quality supervision department shall make a decision on administrative license within 20 working days from the date of accepting the application. In line with the licensing conditions, the quality supervision department shall, within 10 working days from the date of deciding to license, make and issue the Certificate for Special Equipment Operators; Do not meet the licensing conditions, the quality supervision department shall make and serve the "decision not to grant administrative license", notify the applicant in writing and explain the reasons.
