Traditional Chinese medicine has unique advantages in treating gynecological diseases, and it is the key to treat gynecological diseases by using TCM theory based on syndrome differentiation. On this basis, it is more important to choose the right Chinese medicine, which can get twice the result with half the effort. Therefore, this paper analyzes the characteristics of traditional Chinese medicine in treating gynecological diseases according to the order of gynecological classics, belt, fetus, childbirth and miscellaneous diseases. Because of the rich experience in drug use and their own characteristics, only a small part of them can be extracted, hoping to provide enlightenment for clinical treatment.

1 overview of gynecological diseases

According to the physiological phenomena of women's menstruation, leucorrhea, pregnancy, childbirth and lactation, gynecology in traditional Chinese medicine divides gynecological diseases into five categories: menstrual disease, leucorrhea, pregnancy disease, postpartum disease and miscellaneous diseases. Among them? Menstrual disease? Refers to diseases characterized by abnormal menstrual cycle, menstrual period, menstrual quantity, menstrual color and quality, or accompanied by abnormal menstrual cycle, or with obvious discomfort symptoms before and after menstruation; ? Down with the disease? The amount of leucorrhea is obviously increased, and the color, quality and taste are abnormal or accompanied by systemic and local symptoms; ? Morning sickness? Refers to pregnancy-related diseases during pregnancy, also known as? Prenatal diseases? ; ? Postpartum disease? Refers to the occurrence of diseases related to childbirth or puerperium in the new postpartum or puerperium; In addition to menstrual, vaginal, fetal and obstetric diseases, various diseases closely related to gynecological anatomy, physiology and pathology are collectively referred to as? Miscellaneous diseases? This paper mainly discusses hysteria and infertility. Callus refers to a woman with swelling, pain or abnormal bleeding in the lower abdomen cells. Infertility refers to a person who has a normal sexual life and has not been pregnant for one year without contraception.

2 Talk about the characteristics of medication

2. 1 Menstrual diseases

Menstrual disease is the most common gynecological disease. In terms of medication, there are many records in ancient literature, such as Women's Notes. Is there a menstrual door? During menstruation, the medicine of Kuhanxin Powder is prohibited? Warning; Chen Su 'an Gynecology Supplement? Is there a menstrual door? Excessive use of cold medicine should not be used in menstrual regulation? And then what? On the inadvisability of excessive use of pungent drugs to regulate menstruation? The monograph.

There are many kinds of menstrual diseases, such as premenstrual period, menorrhagia, metrorrhagia, dysmenorrhea and amenorrhea. , according to the treatment? Don't make up before, don't attack after? Principles of drug selection. For example, during menstruation, drugs such as angelica sinensis, Ligusticum chuanxiong, peach kernel and safflower are commonly used to promote menstrual blood discharge, but those with menorrhagia should use drugs such as safflower and salvia miltiorrhiza with strong blood circulation and blood stasis removal effects with caution to avoid excessive vaginal bleeding.

Prunella vulgaris [1] can reduce liver fire alone, avoiding the accumulation of uterus due to yin blood during menstruation, and the liver loses its hiding place, which is unfavorable for discharge; Pollen Typhae is used for promoting blood circulation, helping Leonurus japonicus to contract uterus, promoting diuresis and draining water, and relieving menstrual edema; If decocted, remove blood stasis and stop bleeding to prevent excessive blood circulation and blood circulation; For people who are sick in Chongren [2], they should use flesh and blood products to directly enter Chongren, which can nourish qi and blood, yin and yang, roast tortoise plastron and turtle shell to nourish yin essence, velvet antler or antler cream to warm kidney and strengthen yang, and placenta Hominis and donkey-hide gelatin mainly nourish yin and replenish blood. Products that tonify liver and kidney in the later stage, such as Radix Paeoniae Alba, Radix Rehmanniae Preparata, Cornus officinalis, Rhizoma Dioscoreae, Poria, Radix Salviae Miltiorrhizae, etc. , often used. Bupleurum, Radix Curcumae, Folium Citri Tangerinae and Cortex Albizziae are commonly used in premenstrual period to soothe the liver, regulate qi and calm emotions. But pay attention to the dosage of Bupleurum. Although Bupleurum has the effect of soothing the liver, if it is too soothing, it will lead to fire and damage yin and body fluid. The patient's symptoms are tinnitus, head swelling and headache, and conscious hot air flows from coccyx to Dazhui, and then from Fengchi to brain, which makes the brain very noisy and irritable [3].

Less menstrual flow is due to spleen deficiency and dampness. Cortex Mori, Radix Clematidis, pericarpium Benincasae, and Semen Coicis can strengthen spleen and eliminate dampness. Dysmenorrhea is mostly due to kidney-yang deficiency, which leads to pain. Raw ephedra, cassia twig, Evodia rutaecarpa, dried ginger, cinnamon, etc. It is used for warming yang, promoting qi circulation and relieving pain. For patients with vaginal bleeding, cacumen Platycladi, Pollen Typhae charcoal and cuttlebone can be used.

What should we pay attention to when treating menstrual diseases? Tune,? And then what? Mainly. It is not advisable to use pungent and dry products to avoid consuming blood and damaging yin. It is advisable to choose more peaceful products such as Astragalus, Epimedium, Morinda officinalis, Dipsacus and Cuscuta chinensis. Don't use cold ones, such as those with yin deficiency and strong fire, and often fry Cortex Phellodendri and Cynanchum atratum. Are you afraid? Although the heat pathogen is removed, the fire turns cold and the blood stasis remains? ; And don't use blood-breaking drugs too much, avoid excessive attack and cutting, so as not to consume too much gas and hurt blood. Motherwort and Eupatorium adenophorum should be used. Regulating qi should not be too perfunctory, so as not to rob yin and consume gas. Light and delicate products such as rose, bitter orange, green tangerine peel, dried tangerine peel and bergamot can be selected [4].

2.2 Excessive leucorrhea

Taking medicine to nourish the disease, Su Wenxuan's model of treating the original disease advocates the rule of law with pungent, bitter and cold drugs, to relieve depression and to treat dryness with heat, but not with pungent drugs. "Danxi Heart Therapy" suggests that the treatment is mainly dryness and dampness, and those who inject wet phlegm below are the main ones. Xue Lizhai advocates strengthening the spleen and rising yang to stop leukorrhagia.

Fu Qingzhu gynecology? The next article points out? Is everything under the belt wet? Therefore, the syndrome differentiation treatment is mainly based on diuresis, and Wandai decoction, Yihuang decoction, Simiao powder and other prescriptions are commonly used in clinic to clear heat and strengthen the spleen, eliminate dampness and stop leucorrhea. However, Wandai decoction is effective for those with mild symptoms of spleen deficiency and dampness, but ineffective for those with excessive fatigue and sinking of middle qi, which can be replaced by Yiqi Clever Decoction. Those with yin deficiency, damp-heat and internal heat can be treated with Simiao Pill and Artemisia annua Biejia Decoction. Atractylodes macrocephala is easy to use, and its dryness is greatly reduced. Atractylodes macrocephala is rich in fat and has no harm to yin. The combination of the two sides is mainly clear and thorough, nourishing yin but not greasy, eliminating dampness without hurting yin, and eliminating damp-heat and restoring yin deficiency [5]. Because leucorrhea is closely related to dampness, considering that Radix Paeoniae Alba nourishes yin and helps dampness, it is often not used, but Radix Angelicae Dahuricae is used to stop leucorrhea and relieve pain. Some doctors have summarized [6]: To treat leukorrhagia, use more dampness-dispelling drugs, use bitter cold drugs with caution, use less yin-nourishing drugs, and do not use pungent and hot drugs.

In addition to oral administration, Chinese medicine fumigation and washing, with the help of drug volatilization, the effect is good. Commonly used external washing drugs include Fructus Cnidii, Radix Sophorae Flavescentis, Cortex Phellodendri, Herba Taraxaci, Radix Stemonae, dried alum, and Fructus Zanthoxyli. Vaginal dryness accompanied by dried alum plus verbena, salvia miltiorrhiza and licorice; For patients with white lesions of vulva, reduce dried alum and add turmeric and psoralea corylifolia; For chapped and broken skin, reduce pepper, add ebony and terminalia chebula, and apply egg butter [7].

2.3 Pregnancy diseases

Drugs used during pregnancy, such as diarrhea, dredging, removing blood stasis, breaking blood, consuming gas, promoting qi circulation, etc., and all toxic drugs should be used with caution or banned, because they may have a certain impact on the health of pregnant women and the development of the fetus, and even cause miscarriage, abortion or fetal malformation. Therefore, it is of great significance to pay attention to the medication of pregnancy diseases. "Women's Quanfang" pointed out:? Medication should be cool during pregnancy, while cassia twig, Pinellia tuber, peach kernel and mirabilite should not be used lightly. Where the medicine is used, it will stop when the disease subsides, and it will be inexhaustible. This is Dafa? . Regarding the principle of medication, "Medical Zong Jin Jian? Essentials of gynecological psychology:? Clearing heat and nourishing blood before pregnancy is the main thing, as is regulating spleen and dispersing qi, and the third is prohibiting sweating and promoting diuresis, depending on the symptoms of deficiency, cold and heat. ?

Since ancient times? Should it be cold before the fetus? Therefore, clearing heat, regulating spleen and preventing miscarriage are the main methods in early pregnancy, and Scutellaria baicalensis Georgi, Atractylodes macrocephala and Gardenia are commonly used. In the middle and late stage, nourishing yin and preventing miscarriage are the main methods, and Radix Paeoniae Alba, Radix Rehmanniae Preparata, Fructus Corni and Fructus Lycii are used. Although the drugs used are mainly drugs for nourishing yin and astringing yin, they are too bitter and cold, or there are drugs for purgation, such as mirabilite, phellodendron amurense and Alisma orientalis. , should not be selected, in case of excessive yin injury caused by bitter cold, resulting in fetal instability; In addition, heat-clearing drugs should choose drugs with relatively stable taste, so as not to affect the development of the fetus due to supercooling. Flos Lonicerae, Scutellariae Radix and Artemisia annua are commonly used. Should not use angelica, chuanxiong and other pungent temperature? Go and not keep it? Products. Considering the problem of pregnancy reaction in the first trimester, we should be careful to use too greasy drugs, such as jujube and white peony, which will hinder the stomach [8].

For patients complicated with diseases, such as constipation, yam, Radix Rehmanniae, Cistanche deserticola and other products can be used to nourish kidney and produce sperm, moisten intestines and relieve constipation, and drugs such as Poria and Alisma orientalis should be used with caution; For those who have frequent urination at night, raspberries and Alpinia oxyphylla can be used; For patients with low back pain, Eucommia ulmoides, Polygonatum sibiricum, Tzu Chi, etc. Often added. For those whose maternal and fetal blood types are incompatible, Artemisia capillaris is is commonly used to reduce antibody titer, and the effect is very good.

Contraindications to pregnancy, such as Pinellia ternata, Curcuma zedoary, Achyranthes bidentata, Leech, Kansui, Daphne genkwa, Scolopendra, Scorpio, etc. It should be used with caution or disabled during treatment, but it can also be selected appropriately according to the needs of the disease. No reason to die, no reason to die? . However, we should pay attention to [9] when taking medicine: products with moderate efficacy such as Bupleurum, perilla stem, pericarpium Arecae, Fructus Aurantii, Rhizoma Cyperi, Semen Persicae, Cortex Moutan, Radix Salviae Miltiorrhizae, Radix Angelicae Sinensis, Rhizoma Chuanxiong, Oletum Trogopterori and Herba Leonuri should be selected; Drugs for nourishing blood and preventing miscarriage, such as Ejiao, Taxilli and Polygonum Multiflori Radix, should be properly compatible. Strictly control the dose. Half failed? In order to avoid fetal injury. For pregnant edema, skin diuretics, such as poria cocos, lentils, mulberry bark and areca peel, should be selected.

2.4 Postpartum diseases

Although there is no literature to explain whether traditional Chinese medicine can enter milk, from the perspective of safety, it should be assumed that drugs can be transported to milk through the blood-milk barrier and absorbed by infants. Therefore, all products that are extremely pungent and hot, extremely bitter and cold, extremely nourishing, severely reducing profits, consuming blood and gas, returning milk and affecting milk secretion and toxicity should be banned or used with caution. If you use senna leaves, rhubarb and other laxatives, you should stop breastfeeding to avoid the pharmaceutical ingredients entering the milk and causing diarrhea in infants. During the treatment with traditional Chinese medicine, lactating mothers should take medicine immediately after breastfeeding, so as to keep the drug concentration as low as possible in the next breastfeeding [10].

What did the predecessors put forward about postpartum medication? Three prohibitions? It is forbidden to sweat heavily to prevent the death of yang, to fall sharply to prevent the death of yin, and to urinate to prevent the death of body fluids. In other words, you should not sweat profusely when relieving exterior syndrome, and you should not cut excessively when attacking interior syndrome. You should use Qinlian to clear away heat with caution, and cinnamon to warm interior syndrome with caution, which will lead to weak stomach qi after delivery. In addition, breast-feeding is required, so the digested products cannot be reused, such as Sparganium, Rhizoma Curcumae, Malt and Massa Medicata Fermentata [1 1]. Postpartum hypogalactia people can add products that promote qi circulation and dredge collaterals, such as Radix et Rhizoma Rhei, Cao Tong, Semen Vaccariae, Fructus Passepartout and pig's trotters, which can dredge collaterals and increase milk.

2.5 Other diseases

2.5. 1 The Outline of Nourishing Yin records:? Good governance, manage its qi and break its blood, eliminate its food and clear its phlegm, and stop at half failure. They shouldn't strike hard at hell to pay to hurt his righteousness. .

The western medical records [12] pointed out:? Rhizoma Sparganii, Rhizoma Curcumae, and Hirudo are all specific drugs for eliminating phlegm? Ginseng, astragalus and other drugs are used to protect qi and blood, so that blood stasis can be removed without damaging qi and blood. Besides drugs for promoting blood circulation and removing blood stasis, drugs for resolving phlegm and resolving hard mass, such as Fritillaria thunbergii and Endothelium Corneum Gigeriae Galli, can also be combined, as Li Dongyuan said? Good governance, regulating qi and breaking blood, promoting digestion and resolving phlegm? . Combined with drugs such as orange seed and litchi seed, it can dredge qi and blood and disperse stagnation. In addition, on the basis of syndrome differentiation and treatment, drugs such as Xiangfu and Yujin were added before menstruation to strengthen the power of soothing the liver, and after menstruation, liver and kidney were supplemented to promote the filling of blood sea, and drugs such as Radix Rehmanniae Preparata, Rhizoma Dioscoreae, Cornus officinalis, Semen Cuscutae and Fructus Rosae Laevigatae were reasonably compatible [13].

2.5.2 Infertility infertility treatment generally adopts the concept of combining disease differentiation with syndrome differentiation. On the basis of clear diagnosis by western medicine, the treatment should be based on syndrome differentiation of viscera, qi deficiency and meridians.

Ovulatory infertility often takes kidney-tonifying drugs, such as placenta hominis and antler glue. And cooperate with Radix Paeoniae Rubra and Radix Salviae Miltiorrhizae to promote follicular development and maturation. During ovulation, on the premise of tonifying kidney, the application of drugs for promoting qi circulation and blood circulation, such as Herba Lycopi, Herba Leonuri, Pollen Typhae and Radix Cyathulae, should be increased. Radix Bupleuri, Radix Curcumae, Rhizoma Cyperi and Radix Linderae are added for qi stagnation; For patients with severe blood stasis, change Radix Paeoniae Alba into Radix Paeoniae Rubra, and add Radix Cyathulae, Herba Leonuri, Herba Lycopi, Hirudo and Eupolyphaga Seu Steleophaga; Cinnamomum cassia and Evodia rutaecarpa are added for patients with blood deficiency and cold; For those with fever due to blood deficiency, Scutellaria baicalensis Georgi and Cortex Moutan should be added to the farmland [14].

Tubal obstructive infertility is often treated with Rhizoma Sparganii, Rhizoma Curcumae, Olibanum, Myrrha, Prunella vulgaris, Semen litchi, etc. Promoting blood circulation and removing blood stasis, pangolin, earthworm, leech, etc. Activating qi and dredging collaterals, Fructus Passepartout, Semen Vaccariae, seaweed, Fructus Gleditsiae Abnormalis, white mustard seed, etc. Used to disperse and promote diuresis [15]. Traditional Chinese medicine enema can also be used in clinic, and both oral and external use have good effects.

3 abstract

With regard to the selection of drugs for the treatment of gynecological diseases, we should grasp the principles of medication and pay attention to the accumulation of experienced drugs according to the characteristics of menstruation, vagina, fetus, obstetrics and miscellaneous diseases, so as to achieve the purpose of getting rid of diseases and strengthening the body and improving clinical efficacy. Menstrual diseases are generally treated with Siwutang based on syndrome differentiation, but attention should be paid to regulating qi and promoting blood circulation, warming yang and nourishing yin, but products that break qi and attack blood, warm and dry and cool should not be used, otherwise, although the treatment method is correct, it will not achieve the original effect, but will consume human qi and blood. Please follow the treatment of leucorrhea? Use more dampness-removing drugs, use bitter and cold drugs with caution, use less yin-nourishing drugs, and use no pungent and hot drugs? In addition, we should pay attention to the fumigation and washing of external drugs, and adopt products that clear away heat and dry dampness, detoxify and relieve itching. Both oral administration and external use can eliminate pathogenic factors in a short time. Pregnancy and postpartum diseases, according to? Should it be cold before delivery and hot after delivery? The principle is to use products that nourish yin or warm up, and use drugs based on syndrome differentiation. Be cautious or not to use contraindications to pregnancy, but when conditions require it, use it as appropriate and not stick to traditional constraints. Can it be said that it has really been done? Syndrome differentiation and treatment? Requirements. For hysteria, infertility and other miscellaneous diseases, if the symptoms are appropriate, we should boldly use insect drugs, qi-breaking drugs and other slightly bitter products, so as to obtain the maximum curative effect in the shortest time and avoid using some qi-regulating drugs for a long time, which is not only ineffective, but also exhausting.

Because many doctors have rich experience in clinical medication, this paper only talks about a small part of medication characteristics, hoping to inspire everyone through these medication characteristics, attach importance to clinical medication principles and drug selection characteristics, improve the treatment level of traditional Chinese medicine, and produce satisfactory clinical results.

Gynecological traditional Chinese medicine thesis II

Discussion on the regulating effect of traditional Chinese medicine on gynecological menstrual disorder

In gynecological clinic, menstrual disorder is a very common disease, which may occur in women of all ages from adolescence to premenopausal. The types of menstrual disorders include premenstrual, postmenstrual (and amenorrhea) and irregular menstruation before and after menstruation, and patients are often accompanied by endless menstrual bleeding, menorrhagia or oligomenorrhea [1]. In the clinical treatment of menstrual disorder, the gynecology and obstetrics department of our hospital adopted the method of TCM conditioning and achieved good results. The report is as follows.

General data and methods of 1

1. 1 general information. 98 patients with menstrual disorder in our hospital from 2065438+02 to 2065438+03 were selected as the research object. All patients meet the diagnostic criteria of irregular menstruation. They were randomly divided into two groups (49 cases in each group): the western medicine group was between 20 and 43 years old, with an average of (27. 1? 2.4) years old, 20 cases are unmarried and 29 cases are married. The course of disease ranged from 5 months to 8 years, with an average of (1.8? 0.8) years; The age of TCM group is between 2 1 ~ 44 years old, with an average of (26.9? 2. 1) years old, 2 1 unmarried, 28 married, with a course of 6 months to 7 years, with an average of (2.0? 0.7) years. Comparing the general data of the two groups, P & gt0.05, the difference is not statistically significant and comparable.

1.2 method.

Western medicine group: progesterone and estrogen cycle therapy were used, with two menstrual cycles as 1 course.

Chinese medicine group: conditioning with Chinese medicine. On 1 day after menstruation, the patient took Bushen Yangxue decoction (taking 20 g of Angelica sinensis, 20 g of Radix Rehmanniae Preparata, 0/5 g of Rhizoma Curculiginis, 0/5 g of Herba Epimedii, 0/5 g of Semen Cuscutae, 0/5 g of Cistanche deserticola, 0/5 g of Salvia miltiorrhiza, 0/0. Huoxue Tongjing Decoction (Eupatorium adenophorum 10g, Rhizoma Cyperi 10g, Achyranthes bidentata 10g, Rhizoma Chuanxiong 10g, Atractylodis Macrocephalae 10g, Poria cocos 10g, Codonopsis pilosula 10g.

Evaluation of curative effect of 1.3.

1.3. 1 Cure: after treatment, the patient's menstrual period, menstrual volume and menstrual cycle all returned to normal; Improvement: after treatment, menstrual cycle, menstrual volume and menstrual period basically returned to normal; Progress: after treatment, menstrual cycle, excellent period and menstrual period have been obviously improved; Invalid: after treatment, menstrual cycle, menstrual volume and menstrual period have not changed. Total number of valid cases = total number of valid cases-number of invalid cases.

Hemostatic effect of 1.3.2. Vaginal bleeding stops within 3 ~ 5 days, and menstrual flow returns to normal, which is a cure; Vaginal bleeding stopped within 5 ~ 10 days, with menstrual flow.

1.4 statistical method. In this study, the statistical software SPSS 17.0 was used to process the data. T test was used to compare the measurement data, and X2 test was used to compare the counting data, P

Two results

See table 1 for the comparison of hemostatic effects between the two groups. Table 1 shows that the total effective rate of hemostasis in the Chinese medicine group is 9 1.8%, and that in the western medicine group is 73.5%. The difference between the two groups is statistically significant (P < 0.05).

3 discussion

Traditional Chinese medicine believes that the regulation and production of menstruation is mainly related to the qi and blood of zang-fu organs, and whether the meridians are unobstructed will also have a certain impact on menstruation, and menstruation is most closely related to the kidney. Based on TCM syndrome differentiation, menstrual disorder can be effectively corrected and the normal menstrual cycle can be gradually restored by regulating chong and ren, nourishing qi and blood, nourishing liver and kidney [2].

Liqi Huoxue decoction has the function of dilating blood vessels, effectively dredging blood circulation and eliminating congestion; The effect of Bushen Yangxue decoction lies in warming kidney and enriching blood, which can effectively enhance the energy metabolism of the body. After taking the medicine, the patient will have obvious general fever and be refreshed [3]; The function of Huoxue Shu Jing Decoction is to nourish blood and remove blood stasis, warm channels and dispel cold, which can effectively improve uterine blood circulation and promote endometrial shedding. When treating patients with irregular menstruation with traditional Chinese medicine, the physiological functions of the three organs (liver, kidney and spleen) should be adjusted according to the age of the patients. For example, in adolescence, kidney qi is at the beginning, and irregular menstruation can be attributed to the kidney; Irregular menstruation in middle age can be attributed to the loss of liver qi; Menopause can be attributed to the gradual decline of kidney qi [4, 5].

The results of this study showed that the total effective rate of stopping bleeding and regulating menstruation in Chinese medicine group was significantly higher than that in western medicine group (P < 0.05, the difference was statistically significant. It shows that TCM conditioning treatment of gynecological menstrual disorder can significantly improve clinical symptoms, and the treatment effect is remarkable, which is worth popularizing.
