(1) (two empty boxes) Author's name (first name, last name, followed by an English period), book title (italicized, followed by an English period), place of publication (followed by a colon), publisher or publisher (followed by a comma), date of publication (followed by a comma), page number (followed by an English period)

(2) (two spaces) Author's name (first name, last name, followed by English period), article title (article title from "") followed by magazine name (italics, followed by comma), volume number (issue number), publication year, page number and English period.

To insert footnotes and endnotes in word:

(1) Select the position of the footnote.

(2) Click the Footnote menu item in the Insert menu.

(3) Start to enter the footnote text.
