After the applicant applies to join the Party, the Party Committee or the Party branch committee (if there is no committee, a general general party membership meeting will be held) will discuss and determine that the applicant is an activist of joining the Party, and then start to establish and fill in the registration form.

II. Management of Registration Form

The Registration Form shall be filled in and managed by the Party branch where the applicant belongs. Party branches should strengthen the training and education of activists who join the party, guide party groups (or party branches) and train contacts to do regular practical work. After the Party branch receives the applicants who are ready to join the Party, this form shall be kept by the Organization Department (Political Department, Office) of the grass-roots Party (Industry) Committee.

Three. Requirements for filling in the registration form

Fill in the "Registration Form" with a pen or brush, not a pencil, oil pen or colored pen, or pure blue pen water. Fill in carefully, concretely and realistically, so that the handwriting is neat.

Four, the "registration form" to fill in the columns

(1) Fill in the natural information column of the party membership applicant.

This part includes current name, previous name, gender, date of birth, place of origin, education level, family background, my composition, work unit, position, salary level, year of applying for joining the Party, whether I am a member or not, and the name of the training contact person. Be clear at a glance, you can fill it out truthfully. It should be noted that this part should be understood and filled out by the branch, and the applicant does not have to fill it out by himself. If there is nothing to fill in some columns, the content can be left blank, but the word "none" should be marked.

(2) Fill in the column of "My resume (including education)"

Usually it starts at the age of 7 or when you are in primary school, and the starting and ending years should be connected. Be careful not to omit, and write references for important experiences. Due to the limitation of space, it is necessary to write concisely.

(three) "when and where what kind of rewards and punishments" column to fill in.

"Reward" refers to the situation that you are commended by an organization for your good work and study performance. Such as being awarded the honorary titles of model worker, advanced worker, "March 8th" red flag bearer, outstanding Communist Youth League member, and three excellent students, he was commended and promoted. Don't fill in some official duties (such as "NPC deputy") and verbal praise in this column. "Punishment" refers to the disciplinary action or punishment given by a certain level organization because of my mistakes or negligence. Pay attention to the time, place, organization level, reasons for rewards and punishments. If there is no reward or punishment, fill in "None" in this column.

(4) Fill in the column of "My Major Achievements"

Fill in the applicant's performance in political thought, work and study, social life, style and discipline. Fill in the form faithfully and honestly, and write down all the advantages and disadvantages for the party organization to investigate and understand.

(5) Fill in the column "Recommended by Party Organizations or Communist Youth League Organizations"

It is necessary to fill in the form on the basis of the discussion and consent of the party group or the league organization to recommend the applicant as a party activist. The main contents to be filled in are: when will the party group (league organization) discuss; Party groups (league organizations) can be used as the main basis for the identification of activists joining the party; Agree with the concluding comments recommended as party activists. The leader of the party group (the person in charge of the league organization) shall mark the time at the signature, which shall be consistent with the time when the party group (the league organization) discusses and determines the activists. Due to the limitation of space, attention should be paid to neat and concise handwriting when filling in.

The main contents to be filled in are: the time of convening the branch Committee (or branch meeting); The main basis (advantages) for being recognized as a party activist; Agree with the concluding comments identified as party activists. The signature of the branch secretary at the signature office shall be at the same time as the convening time of the branch committee (or branch meeting).

(seven) fill in the column of "training and education of activists joining the party"

This column is filled in by the training contact. After the applicant is identified as a party activist, the Party branch shall designate a training contact person for him. Generally, one or two official party member are responsible for the training, mainly to educate the activists who have joined the Party about the basic knowledge of the Party, improve their ideological consciousness, affirm their achievements in work and study in time through frequent heart-to-heart talks, and point out their shortcomings and efforts. The training contact person should briefly fill in the time of training and education, the main measures and the thoughts, work and study of the activists joining the party in this column. The training contact person at the signing office shall sign and indicate the time.

(8) Fill in the column "Investigation of Party Branch"

According to "Detailed Rules for the Development of party member in China (Trial)", the Party branch conducts an inspection on activists joining the Party every six months. The main contents of the inspection are: (1) whether the political stance is firm, whether it is highly consistent with the CPC Central Committee politically, and resolutely support and implement the party's line, principles and policies; (2) Whether the motivation for joining the Party is correct, whether there is a correct understanding of joining the Party, whether there is a real understanding of the basic knowledge of the Party, and whether the belief in realizing communism and the idea of serving the people wholeheartedly are firmly established; (3) Whether you are loyal and honest to the Party, whether you have established the idea that the interests of the Party are above all else, and whether personal interests are subordinate to the interests of the Party; (4) whether the work is serious and responsible, whether to actively participate in the construction of Socialism with Chinese characteristics, whether to study business knowledge hard, and whether to conscientiously complete the work and tasks assigned by the party organization; (5) Whether the concept of organizational discipline is firm, whether it strictly abides by the party discipline and state laws and various rules and regulations, exemplary abides by social morality, and dares to fight against bad tendencies. The Party branch shall inspect the activists who join the Party once every six months and fill in this column. After filling in, it should be signed by the party branch secretary, indicating the date of filling in.

The main problems that should be paid attention to when filling in this column: (1) The survey opinions should be specific and the length should not be too short; (2) the opinions should be like yourself, not nonsense and cliches. You can write comprehensive opinions in many aspects, or you can write outstanding performance in one or several aspects; (3) The survey opinions should be changed to reflect the characteristics and changes of activists in each period, and do not want an advantage or a disadvantage to end; (4) Don't substitute hope for deficiency in inspection opinions; (5) Pay attention to fill it in on time, and don't memorize it; (6) When reviewing opinions, the handwriting shall be neat and shall not be altered, especially the filling time shall not be changed. If one column can't be filled in, you can occupy the next column or add another leaflet. If the inspection takes a long time, when all the columns are completed, you can attach a registration form and fill it out, but bind it together.
