The first blank is September 2004-September 2000 ... refers to the year of transfer. Fill in the column of primary school location in the second space. If you were a squad leader or squadron leader in primary school, fill in the squad leader or squadron leader, but leave it blank. The reference will be written by the primary school class teacher. If you change the class teacher, the class teacher who taught you in the last year is the reference.

The second line is from September of the transfer year to September of the primary school graduation year. Fill in the transfer school in the second space, with the same position as above. Refer to the same as above.

The third line was written in September-today in the first year of school. The same is true in the back.
