Qingdao Medical Insurance Card for Urban Residents (hereinafter referred to as "Resident Medical Insurance Card") is the electronic certificate for urban residents to participate in basic medical insurance. In order to standardize the use and management of residents' medical insurance cards and safeguard the legitimate rights and interests of the insured, according to the relevant provisions of the Interim Measures of Qingdao Municipality on Basic Medical Insurance for Urban Residents (OrderNo. Qingdao Municipal People's Government 19 1), combined with the actual work, the relevant issues concerning the use and management of residents' medical insurance cards are hereby notified as follows:

First, the "resident medical insurance card" style

The residents' medical insurance card is a magnetic stripe card, with the words "Qingdao Urban Residents' Medical Insurance Card" printed on the front and the words "Social Security Number, Name, Sex, Date of Birth, Date of Issuance, Precautions, Report of Loss, Consultation Service Phone Number, Issued by Qingdao Labor and Social Security Bureau" printed on the back.

Two, the use of "residents' medical insurance card"

The insured can hold the "resident medical insurance card" to the social insurance agencies, street labor and social security service centers, and designated community medical service institutions to inquire about their insurance information.

When the insured person goes to a designated hospital for hospitalization or to a designated community medical service institution for outpatient treatment, he shall hold a card to confirm whether he enjoys the basic medical insurance benefits.

Three. Distribution scope of residents' medical insurance cards

All kinds of urban residents who participate in the basic medical insurance for urban residents in Qingdao have one card for each person.

Application Form for Resident Medical Insurance Card

The "resident medical insurance card" is made by the social insurance agency where the insured person is located. Among them, elderly residents, severely disabled people, non-employees and children who have participated in the basic medical insurance for urban residents according to regulations should bring their ID cards, household registration books and other valid identity documents to the street labor and social security service center of the insured area to apply for a resident medical insurance card (if entrusted by others, the valid identity documents and copies of the agent should be provided at the same time); Students and children of all kinds of schools and kindergartens are responsible for applying for resident medical insurance cards at social insurance agencies in their insured places.

Two. Issuance of residents' medical insurance cards

"Residents' medical insurance card" is reviewed by the accepting unit, and it is completed within 30 working days and put into the hands of the insured.

Before issuing the card, if the insured person is in urgent need of medical treatment, he can bring his valid identity certificate (if he entrusts others to act as an agent, he must also provide his valid identity certificate and a copy of it) and apply for the card at the social insurance agency where the insured person is located. The social insurance agency shall handle it as soon as possible and notify the insured person or his representative in time.

In the process of issuing cards, in case of personal information error or magnetic stripe cross-linking, all card issuing units shall report relevant information to the Municipal Social Labor Insurance Office and change cards in time. If such problems are found after being distributed to insured residents, the insured person or the insured unit shall go through the card replacement procedures at the social insurance agency where the insured person is located; If there are any omissions, the insured unit or the street labor security service center where the insured person is located will review the business card printing information and go through the business card printing formalities at the social insurance agency where the insured person is located.

After the production of "Resident Medical Insurance Card" is completed, before it is distributed to the insured, if the payment place of the insured changes, the insured shall return to the domicile to receive the "Resident Medical Insurance Card". Non-registered social insurance agencies and street labor security service centers should clearly inform cardholders of the exact location of the card.

"Residents' medical insurance card" charges shall be formulated separately.

Report the loss and reissue the resident medical insurance card.

If the insured loses the resident medical insurance card, he should report the loss to the labor and social security department in the shortest time. The 24-hour service telephone number is 12333. Name, ID number or social security number should be provided when reporting the loss. After the loss is reported by telephone, the insured shall bring his valid identity certificate and photocopy (if entrusted by others, the valid identity certificate and photocopy of the agent shall also be provided) to the social insurance agency to formally report the loss and reissue a new card. Among them, elderly residents, severely disabled people, non-employees, children who are not enrolled in schools or kindergartens, students and children from private or private schools and kindergartens go to the nearest social labor insurance agency for handling; Students and children from other schools and nurseries should apply for insurance at the nearest institutions. All social insurance agencies shall issue new cards to the insured within 7 working days.

Updating and Writing of "Resident Medical Insurance Card"

After the residents' medical insurance card is damaged or degaussed, the insured may go through the formalities of renewal or magnetic writing with the damaged or degaussed residents' medical insurance card and his valid identity certificate and photocopy (if he entrusts others to act as an agent, he shall also provide the valid identity certificate and photocopy of the agent) to the management institution determined in Item 3 of this article. All social insurance agencies shall issue new cards or handle magnetic writing procedures for the insured within 7 working days.

(five) "resident medical insurance card"

After the cardholder loses his resident medical insurance card and reports the loss by telephone, but before reporting the loss officially, he can bring his valid identity certificate and photocopy (if he entrusts others to act as an agent, he should also provide the valid identity certificate and photocopy of the agent) to the management institution specified in Item 3 of this article to go through the formalities of hanging up.

The insured shall not use the resident medical insurance card that has been reported to the designated medical institution or cancelled.

6. Cancel the resident medical insurance card

If the insured person terminates the payment of medical insurance for urban residents due to death, cross-regional mobility or identity change as an urban insured employee, the "Resident Medical Insurance Card" shall be cancelled. Among them, if the identity of the insured person is changed to urban insured workers, the Qingdao Labor and Social Security Card will be renewed according to the regulations after the cancellation of the resident medical insurance card.
