I. Scope of application
All kinds of medical and health institutions in our city (including all kinds of enterprises and institutions, economic organizations and social organizations) are engaged in health professional and technical work, and meet the corresponding reporting conditions, as well as demobilized military cadres. Civil servants (including staff of administrative institutions) and retirees are not allowed to participate in the evaluation of professional titles. According to the relevant regulations of the state, professionals from Hong Kong, Macao and Taiwan who are employed in the Mainland, as well as foreigners who hold permanent residence permits for foreigners or residence permits for overseas high-level talents, can participate in the evaluation of professional titles as required.
According to the spirit of the Notice of Chongqing Municipal Health and Health Committee of Chongqing Municipal Bureau of Human Resources and Social Security on the Pilot Work of Decentralizing the Evaluation Authority of Deputy Senior Professional Titles of Health Technology (Yu She Fa [2020] No.77), Eight 3A public hospitals, including the First Affiliated Hospital of Chongqing Medical University, the Second Affiliated Hospital of Chongqing Medical University, Children's Hospital of Chongqing Medical University, Stomatological Hospital of Chongqing Medical University, Yongchuan Hospital of Chongqing Medical University, Chongqing People's Hospital, Chongqing Hospital of Traditional Chinese Medicine, and Chongqing University Cancer Hospital, have carried out the pilot work of decentralization of evaluation authority for health technology deputy senior titles in accordance with regulations, and are responsible for accepting the application and evaluation of health technology deputy senior titles of medical personnel in their own units, labor contracts and labor dispatch.
Second, the reporting conditions
In July, 20021,Ministry of Human Resources and Social Security, National Health and Wellness Commission and state administration of traditional chinese medicine issued the Guiding Opinions on Deepening the Reform of Professional Title System of Health Professionals (No.51issued by Ministry of Human Resources and Social Security), which started the reform of professional title system of health professionals in an all-round way. In 20021year, our city implemented a one-year transition policy according to the principle of combining national standards with municipal standards. At the same time, it is clearly pointed out that from 2022 onwards, the qualifications, qualifications and basic workload conditions for the application and evaluation of senior health titles in our city will be strictly implemented in accordance with the requirements of Ministry of Human Resources and Social Security [202 1]5 1.
June, 5438+October, 2022 10 In order to thoroughly implement the requirements of the national health professional title reform, the Municipal Human Resources and Social Security Bureau and the Municipal Health and Health Commission issued the Implementation Opinions on Deepening the Reform of the Professional Title System of Health Professionals (No.52 [2022] of the People's Society), and the overall requirements for the declaration of senior health professional titles this year shall be implemented according to this document. Details are as follows:
(a) to declare the general title of the city
Declare the title of deputy chief physician and chief physician, and declare it according to the requirements of the reporting conditions in Annex 2, No.52 [2022] of Chongqing People's Society; Declare the title of deputy director nurse and general nurse, and declare according to the requirements of annex 3 of Yuren Shefa [2022] No.52; To declare the titles of deputy pharmacists and chief pharmacists, according to the requirements of the declaration conditions in Annex 4 of No.52 [2022] of Chongqing People's Social Development Agency; To declare the title of deputy chief technician and chief technician, the declaration shall be made in accordance with the requirements of the declaration conditions in Annex 5 of No.52 [2022] of Chongqing People's Social Development Agency.
The length of service is calculated from June 65438+February 3, 20221. (two) to declare the grassroots "directional evaluation, directional use" title.
To declare the senior health title of "directional evaluation and directional use" at the grass-roots level, and to declare the senior health title by the general practitioner at the grass-roots level, it shall be implemented according to the requirements of the application conditions in Annex 6 of No.52 [2022] of People's Social Development Bureau.
The length of service is calculated from June 65438+February 3, 20221. Qualifications This year, the transition policy will be implemented, starting from the time when the corresponding qualifications are obtained (the staff of public institutions must have hired corresponding professional and technical posts). Since 2023, the calculation of the qualification period starts from the appointment of the corresponding title.
(C) on the grass-roots service experience of the "first evaluation" policy.
According to the spirit of the Basic Medical and Health Promotion Law and the Notice on Matters Related to the Provision of Medical and Health Services by Grassroots Practitioners before Declaring and Evaluating the Deputy Senior Professional Title (Wei Yufa [2021] No.2), all medical and health institutions above the second level and other medical and health institutions above the county level (including clinical, traditional Chinese medicine, oral and public health) in the city are being promoted to the deputy senior level. Medical practitioners in medical institutions below the second level and other medical and health institutions below the county level, as well as medical practitioners who are less than 5 years away from the statutory retirement age and are not less than 50 years old, are not included in the above objects and scope for the time being.
After research, it is decided to implement the policy of "evaluation first, then compensation" during the transition period from 2002/kloc-0 to 2023: those who have experienced less than 1 year in primary medical and health services in that year may apply to participate in the qualification evaluation first, and those who pass the evaluation must make up their primary experience before issuing certificates. For those who can make up the grassroots experience in the following year's reporting month, the qualification time is calculated from the date when the jury passes. For those who can't make up the grassroots experience in the following year's reporting month, the qualification time is calculated from making up the grassroots service experience. If the grassroots experience is not supplemented within 3 years after the review, it is necessary to re-participate in the review of deputy senior titles in accordance with the regulations.
(4) Other relevant policies.
1. According to the spirit of the Notice of the General Office of Ministry of Human Resources and Social Security on Doing a Good Job of Professional and Technical Personnel's Professional Titles in COVID-19 Epidemic Prevention and Control (No.23 [2020] of the Human Resources and Social Security Department), the epidemic prevention and control experience of frontline health technical personnel in COVID-19 (hereinafter referred to as "frontline medical personnel", the same below) is regarded as grassroots work experience 1 year, and it is regarded as having completed continuing education credits in that year. The application or review of senior professional titles can be advanced 1 year, and there is no requirement for professional titles and computer application ability, and there is no rigid requirement for papers (clinical treatment, medical records, diagnosis and treatment plans, research and development achievements of key core technologies, epidemiological reports, pathological reports, research and development of drugs and vaccines, product development and application of reagent testing equipment, work summary, psychotherapy and consulting cases in epidemic prevention and control can be regarded as application review of papers). It is not required to participate in the evaluation of local organizations combined with professional examinations, and it is not limited by the proportion of professional and technical posts in the unit. Under the same conditions, it is recommended to give priority to reporting. When evaluating, first-line personnel are given priority under the same conditions, and their performance contributions in epidemic prevention and control are included in the title evaluation. The position tilt policy is only enjoyed once (whether it passes the audit or not).
Front-line medical personnel refer to health professionals and technicians who work in the front line of epidemic prevention and control in COVID-19, and front-line medical personnel who participate in scientific research on epidemic prevention and control such as new detection reagents, antibody drugs, vaccines, diagnosis and treatment programs, virus etiology and epidemiological research. The specific scope shall be implemented in accordance with the Notice of the Joint Prevention and Control Mechanism of the State Council on Implementing Nursing Measures for Medical Staff in the First Line (Guo Fa [2020]10). First-line medical staff receive temporary work subsidies.
2. According to the spirit of "Eight Measures on Encouraging Experts to Contribute to Win the Blockade War of Epidemic Prevention and Control" issued by the Organization Department of the Municipal Party Committee and other departments, those who have won special commendation and awards for epidemic prevention and control in COVID-19 at or above the provincial level will be included in the evaluation channel of special talent titles. The specific declaration matters shall be implemented in accordance with the requirements of the Municipal Office for Vocational Reform.
3. Foreign aid Tibetan health professional and technical personnel, whose academic qualifications and qualifications meet the relevant professional title promotion policies stipulated in the Regulations on the Administration of Foreign Aid Tibet, will be included in the professional title evaluation channel for special talents. The specific declaration matters shall be implemented in accordance with the requirements of the Municipal Office for Vocational Reform.
4. According to the spirit of the Notice on Further Strengthening the Construction of Poverty Alleviation Cadres (Chongqing Committee [2019]15) and the Measures on Revitalizing Rural Talents in Our City (Chongqing People's Social Development [2019] No.44), it is suggested to participate full-time.
Third, the declaration procedure.
Online declaration of senior health technical titles in the city. All units need to log in to the title declaration and evaluation system of Chongqing Municipal Health and Wellness Committee (website: http:/183.230.146.15: 20553), and download the title declaration and evaluation system of Chongqing Municipal Health and Wellness Committee according to the User Manual of Chongqing Municipal Health and Wellness Technology Senior Title Declaration System (downloaded from the homepage of Chongqing Municipal Health and Wellness Committee). Reporting users who have registered on 20 19 -202 1 can log in to the system to fill in the report without re-registering their account information. The declaration of senior titles of health technology shall be carried out in accordance with the following procedures.
(1) Personal statement
1. The applicant shall report to the following units:
(1) Personnel of public institutions apply to their units, which are reviewed and recommended by the competent departments of the units.
(2) personnel of state-owned enterprises apply to their enterprises, which shall be reviewed and recommended by the competent departments of enterprises.
(III) The personnel of non-public units (including demobilized cadres who choose their own jobs) shall be in accordance with the Administrative Measures of Chongqing Municipality on the Evaluation of Professional Titles (Y.R.F. [2021] No.25) and the Notice of the Municipal Human Resources and Social Security Bureau on Doing a Good Job in the Evaluation of the Professional Titles of Migrant Workers in our City (Y.R.F. [2017] No.235).
Among them, if the work unit and files are stored in our city, it is necessary to have a social security payment record of 1 year or more (it can be calculated continuously inside and outside the city); If the work unit is in Chongqing and the files are stored in different places, they need to be insured in our city for more than 1 year (subject to the social security payment record); If the work unit is in a different place (providing social security payment records in different places for more than 1 year), the files should be stored in our city for more than 1 year.
(4) If the personnel of units in Chongqing need to entrust our city to evaluate the professional titles, they should hold a letter of entrustment issued by the superior competent department with the authority to evaluate the professional titles (annex 1), and send it to the corresponding jury for review and acceptance after being approved by the Municipal Professional Title Reform Office.
(5) The person reporting the abnormality shall review and sign according to the procedure. Fill in the "Declaration Form for Abnormal Professional Title in Chongqing" (Annex 2), which will be audited by the Municipal Professional Title Reform Office.
(6) If the service life of primary health care is less than 1 year, and the city's senior medical and health title is declared, it is necessary to apply to the unit where it belongs, and the policy of "evaluating before filling in" can be implemented only after the unit agrees, and a letter of commitment (Annex 3) needs to be filled in.
2. Log in to the title declaration and evaluation system of Chongqing Health and Wellness Committee, fill in the corresponding information online according to the User Manual of Chongqing Health Technology Senior Title Declaration System, and upload the corresponding electronic certification materials.
(2) unit recommendation
The applicant's unit is responsible for guiding its personnel to declare online, carefully examining and verifying the application materials and the originals and photocopies of valid certificates and online electronic documents submitted by the applicant, strictly checking to ensure that the materials are true, accurate and complete, and recommending and reviewing them in combination with the application conditions of corresponding majors and corresponding professional titles. Among them, if the certification materials are photocopies, they must be signed and confirmed by the auditor and stamped with the official seal before they can be uploaded to the Internet.
If the application materials are incomplete and unclear, all units shall notify the applicant to supplement and improve them within a time limit. If the applicant fails to complete the application within the time limit, it shall be deemed as giving up the application. Do not meet the requirements of the corresponding professional and professional titles, shall not be recommended to declare.
Each unit selects the recommended candidates according to the applicant's assessment and performance. Conditional units can formulate the conditions, regulations and methods for the declaration of professional titles of their own units, and recommend them on this basis. If the "expert review" link is set up, the review experts must have corresponding professional and technical qualifications.
All units should make full use of their own information systems, strictly review the applicant's clinical performance materials, and ensure that the applicant's clinical workload is true. In view of the front-line medical staff for epidemic prevention and control in COVID-19, their units should provide their qualification certificates and front-line work performance assessment materials.
All units shall publicize the Chongqing Professional Title Declaration and Evaluation Form (automatically generated by the professional title declaration system), the first-line medical personnel qualification certificate (Annex 4), performance appraisal materials, representative works and other performance declaration materials in public places of the unit for not less than 5 working days, and if possible, in the office platform of the unit. Materials that have not been publicized shall not be used as application materials to declare senior titles.
The declaration and evaluation materials that have no objection to publicity or have been verified to be no problem shall be signed by the person in charge of the unit on the Internet and reported step by step according to the management authority of professional title evaluation.
(3) Department audit
The competent department at the county level is responsible for reviewing the application materials of the personnel of the subordinate units, and the personnel agency at the county level is responsible for reviewing the application materials of non-public organizations and floating personnel, and sending them to the Human Resources and Social Security Bureau at the same level. County Human Resources and Social Security Bureau (zhigai Office) is responsible for reviewing and reporting the application materials of personnel of enterprises and institutions in this area. Those who pass the examination shall be signed online by the person in charge of the unit and submitted to the Municipal Health and Wellness Commission.
The municipal competent department is responsible for reviewing and submitting the application materials of the subordinate units, and the municipal personnel agency is responsible for reviewing the application materials of non-public organizations and floating personnel. Those who pass the examination shall be signed online by the person in charge of the unit and submitted to the Municipal Health and Wellness Commission.
(4) Materials accepted by the Municipal Health and Wellness Committee.
The Municipal Health and Wellness Committee shall accept the application materials and make a good registration in accordance with the prescribed scope, authority and procedures. Do not meet the qualification conditions, do not belong to the scope of acceptance or not submitted according to the prescribed procedures, will not be accepted, and the application materials will be returned online; If the materials are incomplete or the procedures are incomplete, they will be informed at one time, and if they are not corrected within the time limit (the materials that have not been publicized shall not be submitted), it will be regarded as giving up the declaration.
(five) the jury review
The Municipal Health and Wellness Commission's Job Reform Office will organize the evaluation work in accordance with the relevant provisions, and the jury will organize the professional defense of senior titles according to the needs of the evaluation work (the applicant will no longer participate in the defense after the implementation of "combination of evaluation" for deputy senior titles), and the specific matters will be notified separately.
(6) Termination of review
The review will be terminated under the following circumstances. After the review is terminated, the application materials for review will be returned according to the procedures (except the materials used as evidence for verification):
1. Before the evaluation by the jury, complaints and reports were received and verified;
2. In the process of review, it is found that there is fraud, which is verified by investigation.
(seven) the examination and approval results.
Within five working days after the end of the review, the Municipal Health and Wellness Commission Job Reform Office will submit the review to the Municipal Job Reform Office as required, and publicize the review results for a period of not less than five working days. In order to facilitate public access, the publicity information of the evaluation results is submitted to the Municipal Vocational Reform Office for simultaneous publicity. During the publicity period, investigate and verify the clues found by reporting complaints and other means.
After the end of the publicity 15 working days, if it is passed without objection or no evidence is found, it will be notified to the Municipal Vocational Reform Office to confirm the issuance. If it belongs to the entrusted evaluation, it shall issue a notice of the evaluation result to the entrusting unit or reply to the evaluation result.
Within 1 month after the issuance of the Notice of Confirmation of Professional Title Qualification, the reporting unit will forward the Notice of Confirmation of Professional Title Qualification to the reporting unit, and the Municipal Health and Wellness Commission will improve the stamping procedure of the Chongqing Professional Title Declaration and Evaluation Form, and notify the applicant's unit or personnel agency to receive the declaration and evaluation materials. Those who fail to receive it within the time limit will bear the consequences. 1 copy of Chongqing professional title application evaluation form shall be stored in the applicant's file, and 1 copy shall be stored in the unit file.
(8) Review
If the applicant refuses to accept the evaluation results involving himself, he may apply for review and complaint in accordance with relevant regulations.
The main purpose of the review is to check whether the review procedure conforms to the norms and whether there is misjudgment caused by misreading or omission of the applicant's performance materials during the review process.
The applicant for reexamination shall submit a written application for reexamination within 2 months after the reexamination result is confirmed (it will not be accepted after the deadline).
Fourth, the material requirements
(1) The applicant shall fill in the corresponding forms according to the requirements of the declaration system and upload relevant certification materials online. For specific requirements, please refer to the User's Manual of Chongqing Municipal Health Technology Senior Professional Title Declaration System and the Requirements for Submitting Application Materials (downloaded from the homepage of Chongqing Municipal Health and Health Commission's professional title declaration and evaluation system).
(two) in addition to the online application materials, the reporting unit also needs to declare the following paper materials to the Municipal Health and Wellness Committee:
1. List of Applicants for Senior Professional Title of Health () in Chongqing. After the reporting unit submits the application materials to the Municipal Health and Wellness Committee in the reporting system, it will print the List of Chongqing Health () Senior Professional Title Reporting Personnel on the Internet according to the seven categories of the jury and the two levels of high positive and subtropical high pressure, and declare it with the official seal of the unit. The applicant shall uniformly print the roster of all applicants to which the applicant belongs, and separately print the roster of an applicant, which will not be accepted.
2 "Chongqing title declaration evaluation form" in duplicate. After the reporting unit submits the materials to the Municipal Health and Wellness Committee in the reporting system, the reporting personnel will print the Chongqing Professional Title Reporting Review Form (automatically generated by the professional title reporting system) online, and the person in charge of the unit will sign the opinions step by step according to the management authority of professional title evaluation and affix the official seal to the report. Those who have not signed their opinions and stamped their official seals will not be accepted.
3. If it is an exceptional declaration, it is necessary to report the original 1 copy of the Chongqing Professional Title Exception Declaration Form approved by the Municipal Professional Title Reform Office to the Municipal Health and Health Commission for job reform.
4. If the personnel of units in Chongqing need to entrust our city to carry out job title evaluation due to work, they will go to the Municipal Health and Wellness Committee for job reform with the "Letter of Entrustment Evaluation" 1 copy approved by the Municipal Office of Professional Title Reform.
Five, the declaration schedule, acceptance place and contact information.
(1) Time and arrangement for accepting online materials
The opening hours of the professional title declaration and evaluation system of Chongqing Health and Wellness Committee are from 2022 to 2025, 165438+30, of which165438+22-165438+30 only accepts materials corrected according to regulations and does not accept new personnel.

All units are requested to promote the declaration according to the above schedule, and will not be accepted after the deadline.
According to the actual work, the district and county vocational reform offices and relevant departments determine the time schedule for the declaration of professional titles in their respective regions and departments, and the applicants must submit materials in a timely manner in strict accordance with the time schedule of their respective regions and departments.

Disciplinary requirements of intransitive verbs

Seven, the relevant matters needing attention
