First, the pre-test epidemic prevention and control requirements
Candidates who apply for basketball and track and field events in our school need to conduct on-site qualification confirmation and special sports tests. Please consciously do a good job in epidemic prevention and control before the exam.
1. Candidates who have been tested by our high-level sports teams must meet the epidemic prevention requirements of Shanxi Province, Taiyuan City and our school before taking the test. Please pay close attention to the epidemic prevention and control requirements of Shanxi Province, Taiyuan City and our school. See the websites of relevant departments for specific epidemic prevention and control requirements.
2. Candidates should do a good job of personal protection, and conduct self-health monitoring and temperature detection for 14 days before entering school. Please download and sign the Report of Candidates' Health Status (see the attachment for details), send the electronic version of the Report of Candidates' Health Status to gaoshuiping@tyut.edu.cn before June 10 (please name the email with the candidates' names), and bring the paper version to the examination room. Candidates who fail to submit the electronic version or carry the paper version on time are deemed to have given up the qualification for this examination.
3. Candidates need to register Shanxi health code with their mobile phones in advance, and provide Shanxi health code, travel code and 48-hour negative nucleic acid certificate for on-site qualification confirmation and special testing. /kloc-candidates who have lived outside the province within 0/4 days or other candidates involved in the epidemic need to provide proof materials for completing the control measures stipulated by Taiyuan.
Second, on-site qualification confirmation (basketball, track and field candidates)
1. On-site confirmation time: June 2022 16 at 8: 30 am-1:30 am.
2. Site Confirmation Location: Track and Field of Huyu Campus of Taiyuan University of Technology
3. Candidates' on-site confirmation process: All candidates who have passed the qualification examination and publicized on our school's enrollment network will enter the school from the west gate (Qianfeng South Road) of Huyu Campus of Taiyuan University of Technology with their own audit materials and negative nucleic acid certificates within 48 hours before registration, show their Shanxi location code and itinerary code, and have physical examinations such as temperature detection. They will enter from the west gate of track and field in Huyu campus, go through the relevant formalities and get the test admission ticket, and then return to campus by the same way. Parents of candidates are not allowed to stay in school.
4, carrying information details
(1) Paper report of negative proof of nucleic acid test within 48 hours before registration;
(2) The candidates' green codes of "Shanxi Local Code" and "Tourist Card" on June 16;
(3) Necessary certification materials for lifting the control measures;
(4) the paper version of the candidate's health status report form;
(5) The paper version of Basic Information Form for the Registration of High-level Sports Teams in Taiyuan University of Technology; Original ID card and grade certificate for on-site certification;
(6) The grade certificate corresponds to the competition and other original materials such as the competition order book, achievement book and award-winning certificate that meet the registration requirements in recent three years;
(7) Three recent 1 inch bareheaded color photos;
