I. Scope and conditions of the accreditation process
(1) Accepting organs and objects
The following persons who have not reached the statutory retirement age of the country, abide by the Constitution and laws, love education, fulfill the obligations stipulated in the Teachers Law, abide by teachers' professional ethics and have good physical and mental quality may apply to the Minhou County Education Bureau for junior high school, primary school and kindergarten teacher qualifications:
1. China citizens with Minhou county household registration.

1. Teachers in primary schools and kindergartens should have graduated from secondary normal schools and secondary preschool normal schools or have a college degree or above. Other technical secondary school graduates can't participate in the qualification identification of primary school and kindergarten teachers.

The valid academic qualifications recognized as teachers refer to the national education series recognized by the education administrative department of the State Council, which can be inquired in the "China Higher Education Student Information Network" (www.chsi.com.cn) or certified by the "National College Students Information Consultation and Employment Guidance Center". Academic degrees obtained outside the country (territory) can be used as qualified academic degrees to identify the qualifications of corresponding teachers after being certified by the "Study Abroad Service Center of the Ministry of Education".
Second, online declaration matters
Teachers' qualification applications are registered online. Eligible applicants should log in to "China Teacher Qualification Network" (www.jszg.edu.cn) within the online reporting time specified by the accreditation body (see the online reporting time and items below for details), make a declaration from the "online reporting entrance for teacher qualification applicants" and fill in the registration information truthfully. If it is overdue, it cannot be submitted. The applicant should download and print a "personal book" on the online application interface, scan or take photos after signing it himself, and upload it according to the program requirements. After successful registration, you must check the overall effect when previewing the Teacher Qualification Application Form. If you find that the personal book is in an incorrect position and the signature is unclear during the preview, please upload it again.
(1) Online declaration time
In the first half of 200212002, Minhou County Education Bureau applied for teacher qualification online twice according to different types of personnel. Applicants must carefully read the relevant requirements and correctly select the authorized institution, online reporting time period and on-site confirmation point information according to their actual situation. 2. Other applicants, the online reporting time is from June 10 to June 24, 2021; Minhou County Education Bureau is selected as the online newspaper accreditation agency, and Minhou County Administrative Service Center is selected as the on-site confirmation point. (2) Precautions
1. Applicants should carefully read the tips for online application for teacher qualification in China, and fill in the personal application information truthfully and accurately according to the online application process and tips. The types and disciplines of teacher qualifications recognized by the applicant shall be consistent with those shown in the Certificate of Qualification Examination for Primary and Secondary School Teachers. If the information is untrue or inaccurate, the applicant shall bear the responsibility.
2. Special description of photos: the photos submitted by online declaration, the photos posted on the physical examination report and the photos submitted by on-site confirmation must be the same back edition (the standard formal photos of the past three months, with a white background, and myopia does not wear any jewelry and glasses). Any photos that do not meet the above specifications are considered unqualified.
3. The applicant should make a true and correct statement in his personal letter. If it is inconsistent with the facts, the education administrative department will impose an administrative penalty on the applicant according to the relevant national laws and regulations, such as "revoking the teacher qualification and not re-obtaining the teacher qualification within five years" or "losing the teacher qualification and not re-applying for the teacher qualification".
4. All the teacher qualification certificates recognized this time are sent by express mail (free of charge). Please fill in your detailed address (accurate to the house number) and mobile phone number accurately when applying online.
5. After the personal online declaration is completed, you should log in to the online newspaper system again, and check the personal current declaration status, information and matters needing attention during the on-site confirmation. Please pay attention to the personal online declaration and the message information given by the authorized institution.
Third, physical examination
In the first half of 20021,the applicants who participated in the teacher qualification certification in Minhou County underwent physical examination according to the notice of Fujian Province's Physical Examination Standards and Measures for Applicants with Teacher Qualification (revised on 20 18) (Min Teacher [2018] No.20), and the medical examination fee was collected by the hospital and paid by the applicants themselves (WeChat).
Full-time normal graduates from Fujian Normal University, South China Women's Vocational College and Minjiang Normal College in 20021year were organized by various institutions for physical examination. Other types of applicants can choose one person to attend the physical examination at the physical examination center of Fuzhou First Hospital or Minhou County Hospital.
There are a lot of people attending the accreditation this year. Please make an appointment for a physical examination in advance! If it is delayed until the last few days, the consequences will be borne by the applicant!

(2) Medical examination arrangement of Minhou County Hospital
1. Physical examination time: 2021April 27th (Tuesday) to June 26th (the county physical examination center normally goes to work every Saturday morning), and arrives at the physical examination center at 8: 00 am.
3. Make sure that the applicant can contact the physical examination center within 7 working days after the physical examination and get the report form by himself. (Pick it up in the morning on weekdays)
(3) Physical examination process
1. Check-in time is 8: 00 every morning. Physical examination must be on an empty stomach. The applicant carries his/her ID card and 1 inch photo (the photo submitted by the system and the photo confirmed on the spot have the same background color, otherwise it will be regarded as unqualified in the physical examination report), downloads the "Civil Administration APP" on the mobile phone, handles the "Bamin Health Code", wears a mask, and goes to the physical examination center of my choice for physical examination. Go directly to the physical examination department of the hospital to get the physical examination form, fill in personal information and paste photos according to the requirements of the physical examination form, and then participate in the physical examination.
2. Women's physical examination should avoid the physiological period. If they can't avoid the menstrual period, please inform the medical staff before the examination. Items that have no impact can be done first. Mark "Gynecology not checked" on the cover of the physical examination form with a pencil, and tell the front desk staff to remember to leave the medical examination notice and physical examination form, and not to bring them back by themselves. After the menstrual period, take your ID card to the front desk of the physical examination center to get the physical examination notice, and go to the corresponding department to make up the gynecological examination, but the supplementary items should be completed before 202 1 (inclusive).
3. The physical examination report is collected by individuals.
4. In order to improve the conclusion of physical examination, the physical examination institution may add physical examination items to individual applicants, conduct further examination or review the preliminary examination items as needed. The medical examination hospital will usually call the above patients within 7 working days, so please keep the phone open after the initial medical examination.
(4) If there are missing items in the physical examination report or the conclusion is not exact or clear, the teacher qualification accreditation institution may require the applicant to make up the investigation in time at the physical examination center where he is located. The current physical examination report is only applicable to the current teacher qualification identification work. Those who fail the medical examination will be notified directly by the medical examination hospital without further notice.
(5) Failing to complete the physical examination for personal reasons (including failing to complete the chest X-ray examination due to pregnancy, etc.). ), according to the documents issued by the Fujian Provincial Department of Education, you will not be able to issue a physical examination conclusion and you will not be able to apply for this teacher qualification. (Warm reminder: Pregnant women should inform the medical staff in advance that they can't participate in this physical examination, so as to avoid the chest X-ray affecting the fetal development. )
(6) Precautions:
1. Eat a light diet three days before the physical examination, do not drink alcohol, coffee or strong tea, do not eat a lot of sweets, avoid strenuous exercise, and have an empty stomach on the day of the physical examination;
2. It is advisable to dress loosely and lightly, and don't wear a necklace. Women should not wear dresses and pantyhose, especially clothes with metal buttons or sequins, and bras with steel brackets and metal buttons. All hair should be placed on the top of the head;
3. Those who have a history of major diseases (referring to those who have been hospitalized or have undergone major surgery or need long-term medication), please bring relevant medical records, examinations and other supporting materials and inform the doctor of the history of major diseases and traumatic surgery. It is forbidden to resort to deceit or impersonation. If the medical examination results are affected by concealing the medical history, the consequences will be at your own risk;
4. If the vision is less than 4.8 or above, please bring your own glasses that can correct your vision to 4.8 for corrective vision test; Contact lens wearers should inform the staff before vision examination;
5. Fourth, on-site confirmation qualification examination

(2) On-site confirmation of submitted materials

The list of materials should be ticked according to the actual situation, and the rest materials should be attached in order, and the left side should be bound up and down, so that the confirmation points can be submitted when the site is confirmed.

Note: This photo is used for certificate preparation, and it must be a color photo with the same background as the photo submitted online and the physical examination photo, and it must meet the photo requirements of "Precautions" in the second point above.
