Please note that the number of vaccines required by each school is different. For example, in the above two examples, Columbia University only requires two vaccines, while Upenn requires five vaccine records. The more common vaccine requirements are:
MMR leprosy gills generally need 2 stitches (4 weeks apart), or show antibodies through physical examination.
(Measles, Mumps and Rubella)
Hepatitis B generally requires 3 injections (at intervals of 4 weeks), or shows antibodies through physical examination.
Chickenpox Chickenpox generally requires 2 stitches (4 weeks apart), or it is proved by medical records that there is no risk.
DTP of tetanus-diphtheria-pertussis does not require a specific time interval, but it cannot be replaced by TD vaccine. Most students received 4 doses of this vaccine within 1 year after birth.
Meningococcal meningitis usually requires 1 needle.
The vast majority of schools with tuberculosis conduct this kind of test during the foreign student report week, which is called tuberculosis test.
It doesn't matter if students don't know the nature and requirements of these vaccines. All you have to do is take or print the requirements of the school to the local "entry-exit inspection and quarantine center" and give these documents to the international doctors there (the doctors in the center are all qualified), so they will know how to handle it and how to fill out the form. Don't deliberately omit the requirements of the school in order to save money. Official website of each school has made it clear that students who do not meet the immunization requirements of this school may not be able to register normally. In fact, hospitals in American universities also provide registration of these vaccines, but the price is much higher than that in China. Students are advised to get vaccinated in China. These vaccine records are still useful when applying for resident rights (green cards) in the future, which can save a lot of money.
Entry-exit inspection and quarantine centers (or inspection and quarantine bureaus) in different cities have different requirements for physical examination and vaccination procedures, but in general, people should bring the following documents before going:
School immunization requirements and vacuum records.
Your own vaccine history (if you can still find it)
One original and one copy of the ID card.

After vaccination in the entry-exit inspection and quarantine center, students will get this yellow notebook as a voucher, and the medical certificate is a red notebook.
Students must have noticed that some vaccines need two injections, and the interval between the two injections is 1 month, so everyone should prepare for the physical examination as soon as possible to avoid delaying the trip.
