Persons who apply for registration for the first time shall fill in the Application Form for First Registration of Licensed Pharmacists and submit the following materials:
(1) Qualification certificate of licensed pharmacist;
(2) A copy of the identity certificate;
(four) a list of health checks issued by hospitals at or above the county level within 6 months;
(five) the certificate of the practicing unit;
(six) a copy of the business unit's legal business certificate.
A person who has obtained the qualification certificate of licensed pharmacist shall apply for registration to the local drug administration of the province (autonomous region or municipality) in accordance with the regulations, and can only practice as a licensed pharmacist after obtaining the registration certificate of licensed pharmacist. The validity period of licensed pharmacist registration is 3 years, and 3 months before the expiration of the validity period, the holder must go through the registration formalities at the registration institution with the continuing education certificate.
Article 11 To apply for re-registration, the applicant shall fill in the Application Form for Re-registration of Licensed Pharmacists and submit the following materials:
(1) Licensed pharmacist qualification certificate and licensed pharmacist registration certificate;
(two) the examination data of the practice unit;
(3) Registration certificate of continuing education for licensed pharmacists;
(4) List of health examination issued by hospitals at or above the county level within 6 months.
