The annual inspection of forklift license can not be handled by any quality supervision bureau, but only by the quality supervision department at the place where the license is issued or where the employer is located.

According to the provisions of the Examination Rules for Operators of Special Equipment, the National Certificate for Operators of Special Equipment is valid for 4 years. If it is necessary to continue to engage in its work after the expiration of the validity period, it shall, three months before the expiration of the validity period of the certification project, submit an application for reexamination to the issuing department by itself or by entrusting the examination institution. Operators who work across license issuing departments may apply to the original license issuing department for review, or to the license issuing department where the original employer is located. Those who meet the requirements are allowed to pass the second interview, and the issuing department signs the certificate of special equipment operators.

When applying for review, the holder shall submit the following materials:

1, application form for reexamination of special equipment operators;

2. Original operator certificate;

3. Safety education and training certificate issued by the employer or professional training institution during the certificate-holding period (the content and class hours meet the safety technical specifications, 1 copy);

4. The certificate issued by the employer that the operation time of the licensed project has not exceeded 65,438+0 years during the licensing period (unless otherwise stipulated in relevant safety technical specifications, such provisions shall prevail);

5. Certificate issued by the employer that there are no bad records such as illegal operation during the certificate-holding period (1 copy);

6. This year's physical examination report issued by the hospital (when the assessment outline has special requirements for physical conditions 1 copy);

7. If the relevant safety technical specifications stipulate that the reexamination must take the examination, the corresponding examination certificate shall also be submitted.
