According to the requirements of * * * *, the State Council's Opinions on Strengthening Teenagers' Physical Education and Strengthening Teenagers' Physical Fitness (* * * Guo Ban Fa [2007] No.7) and Guangdong Provincial Department of Education's Notice on Doing a Good Job in Physical Education Examination for Junior Middle School Graduates in Guangdong Province (Guangdong Education and Sports Letter [2013] No.44), in order to further implement the guidance of health first. Promote students to master two sports skills, improve the physical examination system for students' graduation entrance examination, effectively improve the physical health level of the majority of young students, and promote the all-round development of students' morality, intelligence, physique and aesthetics. According to the actual situation of our city, the relevant matters concerning the physical examination for junior high school graduates in Foshan City (hereinafter referred to as the "senior high school entrance examination") are hereby notified as follows:

First, the examination object

Foshan junior high school graduates.

Second, the examination time

(1) Sports Examination: April 15 to May 14.

(2) Physical Education Basic Knowledge Theory Examination: May 14, 3:00-3:50 pm.

Third, the examination items

(1) The physical education content of the senior high school entrance examination in Foshan includes three parts: the test results of National Students' Physical Health Standard (hereinafter referred to as the national standard), the test results of physical education class's sports skill evaluation (hereinafter referred to as the physical evaluation) and the test results of sports events.

1. Junior high school graduates' second and third grade national standard examination results.

2. The "physical evaluation" scores of junior high school graduates in Grade Two are: (1) physical education class comprehensive evaluation [attendance in the whole school year, classroom performance, mental health and social adaptation]. (2) Examination of sports skills: [Basketball dribbles back and forth at half-court, basketball shoots for one minute, volleyball passes the wall for one minute, shuttlecock kicks for one minute, football dribbles around the bar for 25m, swimming 100m] Candidates choose one of the above six items for the examination (see subhead [20 15] for details).

3. There are two subjects in the physical examination, namely: (1) required subjects [800m running (female) and1000m running (male)]. (2) Elected subjects [throwing solid ball, three-level continuous leapfrog, standing long jump, skipping rope for one minute, putting shot, one-minute sit-ups, basketball dribbling back and forth at half-court, one-minute basketball shooting, volleyball passing over the wall for one minute, kicking shuttlecock for one minute, football dribbling around the bar for 25m, swimming100m].

(two) junior high school graduates who meet the conditions for exemption from sports events to take the theoretical examination of sports basic knowledge.

Fourth, the calculation method of test scores

(1) The total score of Foshan senior high school entrance examination is 60 points, which consists of 6 points in national standard test, 4 points in physical test and 50 points in physical test.

(2) According to the requirements of the Notice of the Guangdong Provincial Department of Education on Doing a Good Job in the Physical Education Examination for Junior High School Graduates in Guangdong Province (Guangdong Education and Sports Letter [2013] No.44), combined with the actual situation of our city, the plan for the senior high school entrance examination is formulated, and the total score of the physical education examination for the senior high school entrance examination is 60 points, which is included in the total score of high school enrollment.

1. Junior high school graduates, senior two and senior three, 3 points each. Every academic year, those who pass the national standard examination will get 3 points, and those who fail will get 2 points. Each school completes the test and reports the data within the specified time.

(1) Candidates who meet the exemption conditions, who apply for the exam as required and obtain the exemption qualification, add 2 points.

(2) Candidates who don't take the national standard examination for no reason get 0 points.

(3) social candidates: able to provide "national standard" test results, according to the standards of junior high school graduates; Candidates who can provide exemption conditions get 2 points; Did not participate in the "national standard" test, can not provide proof, 0 points.

2 junior high school graduates "physical fitness assessment" 4 points, physical education class comprehensive assessment qualified 1 point, unqualified 0 points, sports skills assessment qualified 3 points, unqualified 2 points.

(1) Candidates who meet the exemption conditions, who apply for the exam as required and obtain the exemption qualification, add 2 points.

(2) Candidates who don't participate in the physical fitness assessment test for no reason get 0 points.

(3) Candidates in the city: those who can provide the "Certificate of Physical Examination" shall be subject to the standards of junior high school graduates; If they can't provide the certificate, they will get 0 points.

(4) Candidates from outside the city: score 1 point if they can provide the comprehensive evaluation certificate of physical education class, score no if they can't provide the certificate, and score 0 if they can't provide the certificate; If you can provide the physical education class assessment certificate of your school, you will get 3 points for excellent grades and above, 2 points for passing grades, no points for failing grades, and 0 point for failing grades.

3. The full score of sports examination is 50 points (compulsory subjects and elective subjects, with an average score of two subjects).

(3) The perfect score of the theoretical examination of sports basic knowledge is 100, and 60% of the total written test scores of candidates are included in the total enrollment scores of senior high schools.

Five, the examination project rules and scoring standards

Six, the provisions of the physical examination exemption, deferment.

(a) junior high school graduates in any of the following circumstances, you can apply for exemption to take the physical examination.

1. Disabled junior high school graduates who lose their athletic ability (disability certificate issued by the Municipal Disabled Persons' Federation) can be exempted from physical examination.

2. Junior high school graduates can't participate in sports activities due to physical reasons (certified by hospitals at or above the district level), and in the junior high school stage, they have gone through the exemption procedures of physical education class and "Students' Physical Health Standards" (issued by the registration form that has gone through the relevant procedures), so they can participate in sports examinations without trial.

3. Junior high school graduates with abnormal physical development (dwarfism and gigantism), severe malnutrition, obesity and deformity (such as spinal curvature, O-legs and X-legs) can be exempted from inspection. The criteria are as follows:

(1) Malnutrition [calculation formula: weight (kg/ height 2 (m2)] ≤ 14.5.

⑵ Obesity [calculation formula: weight (kg)/ height 2 (m2)] ≥ 30.

⑶ Severe spinal curvature: deviation from the center line ≥ 3.5cm. ..

⑷O-leg: the distance between knees is ≥20 cm.

5.x-shaped legs: the distance between the legs is ≥20 cm.

(2) All junior high school graduates who are exempt from the examination of sports events must apply by themselves and provide the above-mentioned relevant supporting materials, which are attached to the registration form after the school has added opinions and sent to the District Education Bureau for examination and approval.

(3) Junior high school graduates who are exempt from the examination of sports events must take the theoretical examination of basic knowledge of sports.

(four) temporary injuries, junior high school graduates can apply for suspension of physical education examination with the certificate of the hospital at or above the district level. Suspend the examination by each district to arrange time for a time. Junior high school graduates who can't take the physical examination after the prescribed time limit can apply for exemption from the physical examination and take the theoretical examination of basic knowledge of physical education with the certificate of hospitals at or above the district level. The test scores of National Standards and Physical Fitness Assessment of candidates who take the theoretical test of basic knowledge of physical education are not included in the range of results.

Seven, organize the implementation

(a) the Municipal Education Bureau co-ordinates and supervises the physical examination of the senior high school entrance examination, unifies the examination time, content and standards, and the district education bureaus organize the examination in accordance with the relevant provisions of provinces and municipalities. Each district shall not arbitrarily increase or decrease the content of sports examination, and strictly follow the relevant provisions of provinces and cities.

(two) the district education bureau should attach great importance to the establishment of local junior high school graduates entrance examination work leading group, strengthen management, to ensure the implementation of funds. All relevant departments and enrollment departments should clearly define their responsibilities, carry out their duties, coordinate the completion of this work, and effectively strengthen the leadership and management of the examination work according to the needs of the work, formulate the implementation plan for the physical examination of junior high school graduates in this district, and report it to the Municipal Education Bureau for the record.

(three) the district education bureau according to the examination work needs to set up sports test sites, must be approved by the examination group of the city examination room. For details, please refer to the Notice on Announcing the Designated Examination Room for the Senior High School Entrance Examination in Foshan (Foji [2010] No.32). The venue, equipment, examination rules and methods required for each project examination must be strictly implemented in accordance with the relevant provisions of the state, province and city.

(4) If each district needs to add examination rooms, please report to the Municipal Education Bureau before March 12. For details of the application criteria, please refer to the Notice on Printing and Distributing the Setting Standards of the Physical Education Examination Room for the Senior High School Entrance Examination in Foshan (Foji [2065 438+00] 15). The examination room can only be added after being audited by the examination room audit team of the city.

(5) The school should report the national standard examination results of junior high school graduates and junior high school students who took the exam on March 18 to the District Education Bureau for the record, and the school will publicize the national standard examination results on March 7 (the publicity time is 5 days) (see Annex 7 for the national standard examination results report).

(6) The 800-meter and 1000-meter running test items in each examination room are tested by electric chronograph.

(seven) each district should formulate emergency plans for special circumstances (such as strong winds and heavy rains). ), and adjust the scheduled examination time according to the actual situation, and report to the Municipal Education Bureau for the record.

(8) In order to strengthen supervision and maintain the discipline of examination rooms, the Municipal Education Bureau has set up an inspection team composed of relevant personnel from the Department of Physical Education and Health and the Admissions Office to conduct patrol inspections of the examination rooms. Each district should also set up a corresponding supervision team to strengthen the supervision and management of the senior high school entrance examination sports. The city and district set up a telephone number for complaints about the physical examination of the senior high school entrance examination and announced it to the public. The telephone numbers for consultation and complaint of the Municipal Education Bureau are 83205063 and 83380500.

(9) Publication and submission of the results.

1. Candidates' sports scores will be publicized by each district before May 13 (the publicity time is five days) and will be reviewed.

2. Before 4:00 p.m. on May 20th, all districts will input the candidates' physical examination results (including national standard test results, physical education class evaluation results and sports examination results) into the information management system of Foshan senior high school entrance examination.

Eight. any other business

(1) The physical education work of the senior high school entrance examination in the municipal schools shall be brought into the unified management of Chancheng District in accordance with the principle of territorial management.

(2) The implementation plan and specific time arrangement of the physical education examination for senior high school entrance examination formulated by each district shall be reported to the Physical Education and Art Department of our bureau before April 6, and our bureau will organize personnel to check the implementation in the examination rooms of each district.

(3) All districts should conscientiously do a good job in summarizing the physical education examination, and report the summary of the physical education examination for junior high school graduates (including text and electronic documents) to the Sports Health Art Department of our bureau before May 3 1. (IV) In order to further implement the spirit of "Document No.7 of the Central Committee" and the Third Plenary Session of the 18th CPC Central Committee "Strengthening physical education class and Extracurricular Exercise to Promote the Physical and Mental Health and Physical Fitness of Teenagers", and conscientiously implement the requirements of the Three-year Action Plan for School Physical Education in Guangdong Province (Guangdong Sports [2014] No.2), from 20 17, our city.

1. Increase the evaluation score of "physical education class motor skills evaluation" in grade one (No.10 [20] of Ford).
