The template is empty because the software is not installed or the template is lost after installation! Copy from another computer to the corresponding location of your own computer! Or simply reinstall!

As for the latter plug-in, you can't install it, either because your version of Rhino is too low to support plug-in, or because there is something wrong with the plug-in!
