The following academic certificates are not accepted:

3. Any altered academic certificate and the name and ID number on the ID card are inconsistent with the academic certificate and changed at the same time;

4. A diploma with a long history (198 1 years ago), a diploma of a party school that has not been electronically registered by the Ministry of Education, and a diploma of a military academy that has not been enrolled in the military.

Third, the application materials

Basic materials

Precautions:

1. If the graduation certificate is lost, it should be issued to the original graduation school. The style of graduation certificate should be the same as that of the original graduation certificate. The name should be clearly defined as "Graduation (or Completion) Certificate of Colleges and Universities", and the contents filled in are basically the same as the original graduation certificate, including study time, educational level and major. , affixed with my bareheaded photo, stamped with the school seal and number. The certificate should indicate the number of the original academic certificate and the words "Certificate is lost, this certificate is hereby issued", and be signed and issued by the current principal of the school. The certificate of higher education has the same effect as the original certificate. (If it is a self-taught diploma, you need to go to the provincial self-taught office to issue a diploma, which will be issued by the issuing school, and the graduate registration form will be copied and stamped with the official seal. )

2. The above basic materials must be provided, and supplementary materials need to be provided according to the academic category and graduation year. Please make preparations as required before submitting your application, so as to avoid going back and forth.

Supplementary material

1. 199 1 years ago should provide:

(1) The academic certificate issued by the school archives must be signed and sealed by the agent with the office phone number attached;

2. To apply for the certification of certificates such as adult "junior college", online education and open education, self-study exam (independent undergraduate course part), you should also certify the "junior college" education, and provide one of the following three materials according to your own situation.

(1) If the prequalification (specialist) has been certified, provide a copy of the China Higher Education Certification Report or a printed network certificate;

(2) Pre-diploma (college) letter with academic information online, providing printed copies of pre-diploma electronic registration form;

(3) Provide the materials required for prequalification (specialist) certification, and certify the specialist at the same time.

3. In the following two cases, the teacher qualification certificate of the holder shall be provided at the same time.

(1) The graduation certificate of adult and network education is "satellite TV education".

(2) The certificate is a self-taught degree of "Self-taught Examination and Communication Training for Higher Normal University" (Note: the certificate application issued in that year will be postponed for 5 months according to the date of issuance).

4. Military academy higher education.

(1) According to the "active service" situation, provide a copy of the military certificate or officer's certificate, a copy of the veteran's certificate or a copy of the job-changing certificate (if the job-changing certificate is lost, please provide a copy of the job-changing approval report in my personal file).

(2) A copy of the enlistment approval letter or enlistment registration form of the enlisted citizen in my file, and a copy of the student registration form or student registration form.

(3) Those who have obtained the military growth cadre education since1992 must provide a copy of the soldier's enlistment approval in my personal file.

(4) Since 1994, those who have participated in the general college entrance examination and obtained academic qualifications must provide a copy of the Approval Letter for Senior High School Graduates from Ordinary Middle Schools to Attend Military Colleges and Universities (the copy must be stamped with the official seal of the archives department). 6. Those who graduate after 200 1 year (including 200 1 year) are not accepted. )

6. 1. If you change your name and register with the public security department, you need to add:

(1) There is a previous name on the household registration book: provide the original and photocopy of the household registration book or household registration certificate that is consistent with the date of birth of the holder and contains my current name and previous name (check the original to ensure that it is consistent with the photocopy without alteration);

(2) There is no previous name in the household registration book: provide a certificate of citizen's name change consistent with the date of birth of the holder (stamped with the official seal of the police station, and provide the original certificate with the signature of the manager and the office phone number).

6.2. If there is a mistake in the name in the school, you need to add:

(1) Apply to the graduate school to correct the wrong name and reissue the graduation certificate.
