Second, the material format requirements:

The name of the unit filled in the application form must be consistent with the legal business opening certificate of the enterprise (the business or production unit submits the drug business or production license and business license, and the user submits the practice license of the medical institution) and the name on the official seal. (Homophones are not allowed)

4. The ID number filled in the first registration application form should be consistent with the copy of the ID card. The name filled in the application form should be consistent with the original qualification certificate and a copy of the ID card.

5. The copy should be made of A4 paper, with complete and clear contents, and shall not be altered. Documents issued by the government and other institutions shall be provided according to the original size.

6. The legal business start-up certificate submitted by the applicant (pharmaceutical business or production license and business license submitted by the business or production unit and medical institution practice license submitted by the user unit) shall be within the validity period.

7. Annual scores of continuing education for licensed pharmacists 15. A licensed pharmacist with two qualifications only needs to learn one. Those who have obtained the qualification for the first time for more than one year shall also submit the annual credits after obtaining the qualification and the continuing education credits of that year.
