I. Time for implementation

This scheme is applicable to 20 15, and the physical education examination is carried out in full accordance with the requirements of the scheme.

Second, the implementation object

(a) Students in grades 7 to 9 must take the process management assessment.

(two) junior high school graduates who apply for ordinary high schools, vocational high schools, secondary specialized schools, secondary normal schools and five-year junior colleges must take the target effect test.

Fourth, the content of the exam

The content of the examination is divided into two parts: process management evaluation (that is, usual scores) 10 and target effect test (that is, summative evaluation scores) 40, and the total score is obtained by adding the two parts. Among them, the process management evaluation is the test result of "National Students' Physical Health Standard" in junior high school, and the target effect test includes three evaluation indexes: physical quality, physical function and motor skills.

(1) Process management evaluation (10 score)

The test scores of junior and middle schools in the National Standards for Students' Physical Fitness and Health are graded according to the relevant requirements of the Standards and the implementation methods, which are divided into four grades: excellent, good, passing and failing, and then converted according to excellent 10, good 9-8, passing 7-6 and failing 5, which are included in the total score of physical examination.

Note: The process management evaluation of junior high school graduates covers the whole junior high school for three years, and it is evaluated according to the sum of the standard test scores (50%) in the year of graduation and the average scores (50%) in other academic years.

2. Test time

The examination time for ninth grade students is arranged in the last semester, and the examination time for other grade students is arranged in May-June every year.

3. Organization of testing

The process management evaluation is organized and implemented by the school according to the requirements and standards. Test results must be publicized in schools and classes for not less than 5 days. The test results should be reported to China Student Physical Health Network on time and archived.

(2) Target effect test

Target Effect Test According to the basic requirements of the National Standards for Students' Physical Fitness and Health, combined with physical education (health) courses, three indicators of physical fitness, physical function and motor skills are selected to conduct a final test for students.

3. Test scores

The results of the 20 15 senior high school entrance examination will be announced on the spot, and the test results will be posted on the wall in paper form within 24 hours, which will be supervised by candidates and parents.

Verb (abbreviation for verb) exempts the test and postpones the test.

(1) Exempted from testing

(1) Students who have lost their athletic ability due to disability must hold a disability certificate when applying for exemption. After going through the relevant formalities, their sports scores will be included in the total score of entering a higher school according to 70%(35 points) of full marks.

2. physical education class students who have been exempted for a long time due to injury may apply for exemption (certified by hospitals at or above the county level). After going through the relevant formalities, their sports scores will be included in the total score of further studies according to 60%(30 points) of full marks.

Note: Students who can't attend the physical examination due to injury and illness must be publicized in schools and classes before the exam (the publicity time is not less than 15 days). When registering for the senior high school entrance examination, the school shall go through the formalities of exemption from examination at the district senior high school entrance examination center with the Application Form for Exemption from Examination, the certificate of hospitals at or above the county level or the disability certificate.

(2) The individual items in the target effect test shall not be exempted.

(2) Postpone the trial

Students who can't take the exam at that time due to temporary injury, menstrual period of girls and other reasons can apply for an extension of the exam.

Note: Candidates who apply for extension should be led by our school to register for the registration group. The relevant personnel of the test center shall go through the formalities of postponing the trial students on the spot. Make-up time, in principle, arranged within one week after the end of the physical examination.

Sixth, the examination work

(1) Publicity

Do a good job in publicity, widely publicize the purpose and significance of the physical education examination, the relevant provisions and methods of the examination, so that parents and candidates can eliminate their ideological concerns. Do a good job in mobilizing school teachers, especially class teachers and physical education teachers, and provide necessary conditions for candidates to prepare for the exam. In addition, we should also strive for the support and cooperation of relevant social departments and news organizations to do a good job in publicity.

(2) Registration

Graduates who apply for ordinary high schools, vocational high schools, secondary specialized schools, secondary normal schools and five-year junior colleges shall be registered at the district entrance examination center by the school and town office and go through the relevant formalities; Previous students and candidates who have returned to their hometown by borrowing must bring the National Student Physical Health Standard Registration Certificate (junior high school) to the district entrance examination center to sign up for the physical examination. The examination time and place shall be arranged by the district entrance examination office.

By the recruitment center and the recruitment supervision office, PE teachers with high political quality, excellent professional quality and fair handling are selected as testers. The senior high school entrance examination center will train the selected sports testers, and strictly guarantee that they will not be trained and will not be employed if they fail the training.

(4) Setting of test sites

Based on the principle of serving candidates, facilitating candidates and concentrating nearby, the test sites are located in Zunyi No.5 Middle School, Zunyi No.16 Middle School, Aerospace Middle School, Yifu Primary School, Jintang Middle School, Sidu Middle School, Loushanguan Middle School and Tuanze Middle School. Before the examination, the Education Bureau will check the examination venue, equipment and safety, and cancel the test sites that fail to meet the standards.

(5) Organizing the implementation of process management evaluation.

1. The principals of each school are the first responsible person for the management evaluation of the physical examination process in our school. They must organize the work of the national students' physical health standard test, ensure the fairness, impartiality and openness of the test, publish the test results and accept the supervision of teachers and students.

2. Before each test, each school should carefully formulate the evaluation test plan for sports process management, determine the test time, testers and venues, and report to the Basic Education Section of the District Education Bureau, so as to organize personnel to supervise the test process.

3. The school medical staff and security personnel are responsible for the medical, discipline and safety work in the examination room. Physical education teachers are responsible for the preparation of venues and equipment, and the setting of venues and equipment should meet the relevant requirements. Before and during the exam, we should always check the safety of the venue and equipment to ensure the smooth progress of the exam.

4. According to the examination plan, the Education Bureau organizes personnel to inspect the examination center, and parents of Grade 2 students must be appropriately selected to supervise the examination process at the examination center.

(six) to organize the implementation of the target effect test

1. In order to ensure the authority and seriousness of "physical education class Examination", we must strictly implement the examination system, discipline and supervision mechanism, and severely crack down on all kinds of disciplinary incidents.

Seven, examination style and discipline

1. In order to ensure the authority and seriousness of physical examination, we should strictly implement the examination system, discipline and supervision mechanism, and strictly implement the avoidance system of educational examination. Candidates from the immediate family of the tester and school candidates should leave the test site during the test. Examiners, testers and invigilators who violate the examination discipline in the examination work shall be disqualified after verification, and shall be investigated for responsibility with reference to the relevant provisions of the Measures for Handling Violations in National Education Examinations issued by the Ministry of Education.

2. In the course of the examination, if it is found that the candidates have acts such as impersonation and fraud. The total score of physical examination is calculated to be zero.

We should pay special attention to the examination of candidates' cardiopulmonary function, completely eliminate hidden dangers, educate students and remind parents to seek truth from facts, and neither conceal nor lie about their illness; According to the characteristics of physical education examination, scientific guidance should be strengthened to effectively prevent accidental injuries.
