Citizens applying for changing their names shall hold the following documents.

1, the applicant applies in writing (1under 6 years old, the guardian applies);

2. People whose parents divorced and changed their surnames to less than 16 need to provide divorce proof procedures and have an agreement signed by both parents.

3. Household registration book and resident identity card;

4. Those who belong to the staff of organs, organizations, schools, enterprises and institutions shall submit the change certificate approved by the unit and the personnel department of the school; those who belong to students shall submit the change certificate approved by the school; those who have no units shall be issued by the neighborhood (village) committee, and then the certificate shall be signed by the police in charge of the place where the household registration is located, and the change correction form shall be printed with the consent of the director in charge of the police station (/kloc-people under 0/6 years of age change their names without printing.
