4. 1 Basic requirements

The line leader shall conduct dynamic HSE performance appraisal for subordinates and fill in the monthly personal HSE performance appraisal statistics of the company. 4.2 Calculation formula

4.2. 1 The monthly HSE performance of employees is basically divided into 10. The assessment results are used as HSE indicators for the performance appraisal of all employees in the company.

4.2.3 Annual employee HSE performance = annual employee HSE performance appraisal coefficient x annual employee HSE performance weight.

4.3 Employee Deduction Standard

4.3. 1 If an employee in a management position commits any of the following acts, 1 point will be deducted each time (everyone in each position):

1) Failing to complete the HSE work arranged in the personal annual HSE action plan;

2) Failing to attend HSE training on time, failing to complete annual HSE training or violating training discipline;

3) Failing to organize the HSE meeting on time or not attending the HSE meeting for no reason;

4) The line leader fails to organize the formulation of the HSE training matrix for his subordinates, or fails to organize the HSE training for employees according to the training matrix;

5) The line leader did not participate in the HSE audit of the business system in charge;

6) The HSE management system in charge of business scope is not perfect or applicable;

7) Safety behavior observation and communication activities have not been carried out;

8) No safety warning signs are set on the construction site;

9) Other behaviors that fail to perform HSE duties and violate relevant HSE regulations, which have not yet caused consequences.

4.3.2 If any employee in a management position commits any of the following acts, 2 points will be deducted each time (each post and everyone):

1) production organizer, person in charge of site construction, HSE supervisor, etc. Failing to regularly arrange deployment, inspection and hidden trouble investigation of HSE work within the business scope;

2) Arrange personnel without valid operation certificate to engage in special operations, or arrange personnel who have passed the examination without training (or have not obtained corresponding qualifications) to take up their posts or work alone before the expiration of the internship period;

3) HSE facilities cannot operate normally;

4) Employees use vehicles and ships that do not meet safety standards during commuting and shift work;

5) There are no operating procedures within the business scope of the supervisor or the operating procedures are not perfect or perfect;

(6) Failing to take charge of the business field and systematically carrying out hazard identification and risk control;

7) The contractor's management fails to implement unified HSE standards;

8) Full-time HSE management personnel are not in place, and offshore construction sites for drilling, downhole operations and key offshore projects are not equipped with HSE management and supervision personnel;

9) Life-saving and fire-fighting facilities on platforms, ships and offshore construction sites are not complete;

10) Well control equipment and pressure test are not designed and installed for drilling and downhole operations;

1 1) The construction site fails to reserve emergency supplies and personal protective equipment as required;

12) Before the operation, the person in charge of on-site construction did not disclose the risks existing in the operation and the preventive measures formulated to the relevant personnel involved in the operation, and ensured that they had understood them;

13) Failing to rectify HSE problems and hidden dangers found within the business scope in charge according to the rectification cancellation system, and verifying the rectification results;

14) did not give priority to health and safety environment in work decision-making;

15) Failing to stop and correct others' illegal behaviors according to regulations;

16) The special equipment, safety accessories and instruments in charge of the business field have not been regularly inspected or entrusted to a professional testing institution not recognized by the company for testing;

17) failing to wear labor protection articles as required;

18) emergency drills were not organized as required during the construction.

4.3.3 If the manager commits any of the following acts, 5 points will be deducted each time (for each post and everyone):

1) Organizing the construction without effectively examining and approving the construction scheme and design;

2) The person in charge of the construction site who is engaged in cross-operation, joint operation, unconventional operation, dangerous operation and entering new operation areas and using new technologies, tools and new technologies fails to conduct pre-operation safety analysis, fails to apply for or effectively approve the work permit, fails to disclose HSE measures and fails to conduct pre-operation HSE inspection;

3) Command and force others to operate illegally;

4) The toxic and harmful gas detection and fire alarm systems of the platform and the ship cannot operate normally;

5) Stop using HSE equipment and facilities without authorization or HSE equipment and facilities cannot be used;

7) Failing to rectify the hidden dangers of accidents or take effective control measures, and continuing to organize construction operations;

8) Failing to report, analyze and deal with accidents, omens and events truthfully and timely;

9) Organizing the transportation of explosives, radioactive sources and dangerous chemicals in violation of regulations;

10) Smoking in inflammable and explosive places or non-smoking places;

4.3.4 If an employee at an operating post commits any of the following acts, 1 point will be deducted each time (everyone at each post):

1) Did not use the post HSE checklist for post HSE inspection, and truthfully filled in the inspection records;

2) Failing to maintain and maintain the equipment and tools of this position as required;

3) The workplace is messy, and tools and raw materials are not placed correctly;

4) Failing to wear labor protection articles as required;

5) Failing to participate in safety activities, post hazard identification and risk control as required;

6) Failing to set safety warning signs in the workplace as required;

7) Do not participate in HSE training organized by enterprises;

8) Violation of relevant HSE regulations, which has not caused consequences;

9) Engaged in activities unrelated to post work during the post.

4.3.5 If the operator commits any of the following acts, 2 points will be deducted each time (for each post and everyone):

1) Engage in unconventional and dangerous operations without work permit and isolation lock;

2) Failing to discover, rectify and report the HSE problems and accident hidden dangers existing in the post in time;

3) No operating rules or operation in violation of operating rules;

4) Close, block or occupy fire exits (safety exits) or emergency exits;

5) There is no special person to command and supervise the hoisting operation;

6) Failing to use personal safety and labor protection articles at work according to regulations;

7) Incorrect switch status of equipment, facilities, devices and tools for post management;

8) Connect wires or hand-held power tools independently, or have no leakage protection device;

9) Industrial gas cylinders do not meet safety standards or storage and use do not meet safety regulations;

10) failed to report the accident signs and events of this position in time.

4.3.6 If the operator commits any of the following acts, 5 points will be deducted each time (for each post and everyone):

1) engaged in special operations without a valid operation certificate;

(2) transporting and storing explosives, radioactive sources and dangerous chemicals in violation of regulations;

3) Off-duty, sleeping on duty, and working after drinking;

4) Use non-explosion-proof tools, electrical appliances and electronic equipment in inflammable and explosive places;

5) Smoking in inflammable and explosive places or non-smoking places;

6) Violation of operating rules and HSE management regulations, resulting in accidents and dangers.

4.3.7 Deduct 10 for the following results:

1) In case of illegal command and operation, resulting in a general accident of Class C production safety, those who are directly responsible for the accident and those who are mainly responsible for the leadership;

2) Those who command and operate in violation of regulations, causing general B-level production safety accidents and general environmental pollution accidents, and have important leadership responsibilities for the accidents.

4.3.8 Deduct 50% of the annual HSE performance in case of the following results:

Command and operate in violation of regulations, resulting in general B-class production safety accidents and general environmental pollution accidents, those who are directly responsible for the accidents and those who are mainly responsible for the leadership.

4.3.9 All annual HSE performance will be deducted due to the following results:

In case of illegal command and operation, resulting in general A-level and above safety production accidents and major and above environmental pollution accidents, the personnel who are directly responsible for the accidents and the personnel who have major or important leadership responsibilities.

4.3. 10 traffic violation deduction standard

1) Personnel who commit the following acts will be deducted 1 minute each time:

A) Driving the company's vehicles without observing traffic rules, such as traffic signs, markings and signal lights;

B) Driving a company vehicle, with the overspeed below 20% (including 20%).

2) Personnel who commit the following acts will be deducted 5 points each time:

A) Driving a company vehicle without obtaining the company's internal driver's license;

B) Driving a company vehicle, exceeding the speed limit by 20% to 50% (including 50%).

3) Personnel who commit the following acts will be deducted 10 every time:

A) Driving a company vehicle without a valid driver's license;

B) Driving a company vehicle at a speed exceeding 50%.

4.4 Employee reward standard

4.4. 1 reward for employees who commit the following acts 1 point:

1) HSE rationalization proposal was adopted by grass-roots units;

2) Correct others' unsafe behaviors;

3) Found HSE hidden danger in other posts.

4.4.2 Employees who have the following behaviors will be rewarded with 3 points:

1) HSE rationalization suggestions are adopted and promoted by offshore companies and subordinate units;

2) Grass-roots units find and confirm major HSE hidden dangers in other positions.

4.4.3 Employees who commit the following acts will be rewarded 10:

Take effective measures to avoid accidents when eliminating hidden dangers or accident signs that may lead to accidents in time, which shall be confirmed by the subordinate units of the company.
