"Nurse Practice Registration Application Approval Form" (please collect it from the government affairs hall or download it from the municipal government affairs open network, and print two pages on both sides).

Proof of identity

The applicant's identity certificate (copy of the original of the county health bureau).

Academic certificate and internship certificate

The applicant's academic certificate (original and photocopy) and the clinical practice certificate in professional study (the original and photocopy are required for the practice manual or valid clinical practice certificate issued by the medical institution).

Test report card and qualification certificate

Report card and qualification certificate of nurse qualification examination (original and photocopy).

Employment certificate

Relevant materials of the medical and health institution to be employed (employment certificate).

A copy of the original license of the medical and health institution to be employed; (provided by the school).

Application time

The application for registration of nurse practice shall be filed within 2 years from the date of passing the nurse qualification examination; If the application is filed within the time limit, it is necessary to provide 6 months of nursing training and assessment certificate from medical institutions above the second level.
