What is the yellow card of Chengdu University of Information Science and Technology?
The academic yellow card of Chengdu University of Information Science and Technology is a warning way for students not to complete the credit requirements according to the regulations.

According to the Regulations on the Management of Undergraduate Student Status in Chengdu University of Information Science and Technology, students whose grades are below 14 credits per semester, or whose required grades are over 12 credits, will be given a yellow card warning (excluding graduation grades).

Chengdu University of Information Science and Technology has the following regulations on undergraduate student status management

Chapter one? general rule

Article 1 In order to protect students' learning rights, standardize the management of student status, maintain the order of school education and teaching, establish a good style of study and school spirit, and continuously improve the quality of education, these Provisions are formulated in accordance with the Regulations of the Ministry of Education on the Management of Students in Colleges and Universities (Order No.21of the Ministry of Education) and in combination with the specific conditions of our school.

Chapter two? Admission and registration

Article 2 Registration of new students. Freshmen must hold the admission notice and the relevant documents stipulated, and go to school for admission procedures on schedule. Those who cannot arrive at school on time for some reason must ask for leave from the school. Holidays should generally not last more than two weeks. Those who fail to ask for leave or ask for leave overdue shall be deemed to have waived their admission qualification except for justified reasons such as force majeure.

Article 3 After new students enter the school, the school shall conduct an audit in accordance with the national enrollment regulations within three months. Those who pass the second interview will be registered and obtained the status of our school. Those who fail to pass the re-examination will be dealt with by the school as the case may be, until the admission qualification is cancelled.

Anyone who practices fraud or engages in malpractices for selfish ends to obtain a student status, once verified, will immediately cancel his student status. If the circumstances are bad, the school will ask the relevant departments to investigate.

Article 4 Freshmen suffering from diseases who have been diagnosed as unsuitable for studying in the school by the second-class hospitals designated by the school health department (the same below) may apply for retaining their admission qualification for one year. Those who retain admission qualifications do not have a student status. After treatment and rehabilitation, you can apply to the school for admission. The school will designate a hospital for diagnosis and meet the requirements of physical examination. After the second interview, you can re-apply for admission. Those who fail the re-examination or fail to go through the admission formalities within the time limit will be disqualified.

Article 5 semester registration. At the beginning of each semester, students must report to their college with their student ID cards according to the registration date stipulated by the school. In the autumn semester of each academic year, students can only register after paying the fees due for that academic year. Students who are unable to pay the fees due for this academic year due to special reasons may apply on their own, and after being approved by the school, they will go through the registration formalities of suspension from school. The period of suspension of registration is three months. Those who are not registered (including those who are not registered after the suspension of registration) shall not participate in any educational and teaching activities of the school, and shall be deemed to have given up their school status and shall be treated as automatic withdrawal.

Article 6 All students who cannot report to the school on time for some reason must ask for leave in advance from their college, and the registration can be postponed with the approval of the college leaders. The time for suspension of registration shall generally not exceed one week. Failure to report for duty without reason or leave without approval, and failure to report for more than two weeks (including two weeks) shall be regarded as abandonment of school status and automatic withdrawal.

Article 7 The college where a student belongs shall affix a registration stamp on the student ID card of a registered student. If the student ID card is not stamped with the registration stamp, it shall be deemed as unregistered.

Chapter three? Educational system and the longest educational system

Article 8 Academic system of undergraduate majors (hereinafter referred to as "academic system"): The academic system of various undergraduate majors set up by the school is four years.

Article 9 The maximum length of study for undergraduate majors (hereinafter referred to as "the maximum length of study") is four years plus two years, that is, the maximum length of study for undergraduates (except those who join the army) is six years.

Article 10 The school encourages students with strong self-study ability and spare capacity to make their own study plans in advance under the guidance of the college. Anyone who meets the conditions for early graduation may apply for early graduation. Chapter four? Course examination and record method

Article 11 Schools implement the credit system. Credit is the calculation unit of students' learning volume in courses and other education and teaching links. All kinds of courses, education and teaching links and some extracurricular activities set up by the school need certain credits. The specific credit structure and total credit requirements of each major are subject to the talent training plan.

Twelfth courses for students of various majors are divided into compulsory courses and elective courses. Compulsory courses refer to the courses and practical teaching links that students must take according to the talent training plan. Elective courses refer to courses that students can choose independently according to their own interests, abilities and the needs of discipline and professional development according to the provisions of the talent training program.

Thirteenth students should participate in all educational and teaching activities (such as class, military training, internship, etc.). ) the time specified in the talent training plan. For students who are absent from class for no reason, the school will give criticism and education according to the "Regulations on Disciplinary Actions of Chengdu University of Information Technology" and according to the time and circumstances of absenteeism, until disciplinary action is given. The examination of students' qualifications refers to the Measures for the Administration of Undergraduate Examinations and Grades of Chengdu University of Information Technology.

Fourteenth students' courses and education and teaching links must be assessed. Curriculum assessment methods are mainly divided into examination and assessment, which are determined by the talent training plan and curriculum outline.

Article 15 The total evaluation score of the course consists of the final score and the usual score, and its proportion is determined according to the course syllabus. Students pass the course examination and get the required credits; If you don't pass the grade, you won't get credit. Make-up exams will be arranged for courses that fail the exam (unless there are special provisions).

Article 16 Anyone who is absent, unqualified or cheats in the exam without authorization will be marked as absent, forbidden or in violation of discipline, and his qualification for make-up exam will be cancelled.

Seventeenth students who want to improve the assessment results of courses with credits can apply for a second interview, and the management methods of fees, study and examination are the same as those of the second interview.

Eighteenth make-up exam results display column marked "make-up exam", retest results display column marked "retake".

Article 19 Schools measure students' learning by the average grade point, which is calculated by the course grade point and credits. According to the scope of use, it is divided into two categories: average credit grade point and compulsory course average credit grade point.

(1) course grade point. Course grade point is the conversion of students' assessment results in this course.

Course grade point =0. 1× (course grade -50)

Note: This formula is applicable to percentile scores, and other percentile scores should be converted into percentile scores for calculation. Below 60 points, the course grade point is 0.

(2) GPA. The average credit grade point is the weighted average of all course credits.

Note: Subscript I indicates the i-th course, and n indicates the number of courses.

Article 20 Students may apply for exemption from listening to some courses (except political theory courses, physical education class courses and experimental courses with separate courses) if their credit grade points were greater than 3.5 in the last semester, or because the time of selected courses conflicts. The course flow is as follows: I fill in the application form for free lectures in Chengdu University of Information Science and Technology; This course undertakes departmental audit; The teaching secretary of the course undertaking department informs the teacher; School academic affairs office for the record.

Students who attend the free listening course must submit their homework on time and participate in practical teaching and evaluation at all stages. Otherwise, teachers can refer to the Measures for Undergraduate Course Assessment and Achievement Management of Chengdu University of Information Science and Technology to determine whether students are qualified for assessment.

Twenty-first students' ideological and moral evaluation is based on the Code of Conduct for Students in Colleges and Universities (J [2005] No.5), and is conducted once a year in the form of personal summary and democratic evaluation of teachers and students in accordance with the Measures for the Assessment of Moral Education for School Students.

Article 22 physical education class is a compulsory course for students, and its scores are comprehensively evaluated according to attendance, in-class teaching, extracurricular exercise activities and the test scores of "College Students' Physical Fitness and Health Standards" (No.2014). Students who are not suitable for attending physical education class due to physical reasons may apply on their own. After being audited by the school health department and the sports department, they may take part in sports health care classes and be assessed separately.

Twenty-third undergraduate students to participate in the graduation thesis (design) teaching, qualification examination and graduation thesis (design) performance evaluation of the specific measures in accordance with the "Chengdu University of Information Technology undergraduate graduation thesis (design) management approach" implementation.

Chapter five? Transfer from major to school

Article 24 Changing majors should be planned by the school when the teaching resources of the school allow and the students are willing. The conditions and procedures for changing majors shall be implemented in accordance with the Administrative Measures of Chengdu University of Information Technology for Undergraduates Changing Majors.

Twenty-fifth students to major, should be carried out in accordance with the requirements of professional personnel training program. Students should make up the required courses that they have not learned in the lower grades. The credits and achievements of students in the original professional courses can be partially or fully recognized after being transferred to the college and audited by the Academic Affairs Office.

Twenty-sixth students should generally complete their studies in this school. If you can't continue studying in our school due to illness or special difficulties, you can apply for transfer. The transfer conditions and procedures shall be implemented in accordance with the Measures for the Administration of Undergraduate Students' Transfer in Chengdu University of Information Technology.

Chapter six? Undergraduate and minor courses

Twenty-seventh schools in accordance with the relevant provisions of the Sichuan Provincial Department of Education, select outstanding junior college students for undergraduate study. For the specific selection process, please refer to the management method of "upgrading from junior college to undergraduate college" in Chengdu University of Information Technology.

Twenty-eighth college students should be in accordance with the requirements of undergraduate professional training program, after two academic years of study. On the basis of the course syllabus, the course credits and achievements obtained by students in the junior college stage can be recognized in part or in whole with the approval of the college and the record of the Academic Affairs Office.

Twenty-ninth schools implement the undergraduate major and minor system, which encourages some outstanding students to choose the second major (minor major) on the premise of ensuring that their major (major major) is completed. Those who meet the requirements of the school can get a minor certificate. For specific measures, please refer to the Implementation Measures of Minor for Undergraduates in Chengdu University of Information Technology.

Chapter seven? Suspension and resumption of classes

Thirtieth students can complete their studies by stages within the longest academic year. Students should drop out of school in any of the following circumstances:

(a) confirmed by the designated hospital, need to suspend treatment;

(two) leave, absence (non absenteeism) accumulated to one-third of the total semester hours;

(3) For special reasons, I applied or the school thought it necessary to leave the scholar.

The suspension process is implemented in accordance with the Administrative Measures for the Change of Undergraduate Student Status in Chengdu University of Information Technology.

Thirty-first students apply to join the China People's Liberation Army (including the Chinese People's Armed Police Force), and the school will retain their student status until two years after retirement. For the process, according to the "Chengdu University of Information Technology undergraduate student status change management approach" implementation.

Thirty-second students should apply for resumption of school after the expiration of the suspension period. The procedures for students to resume their studies shall be implemented in accordance with the Administrative Measures for the Change of Undergraduate Student Status of Chengdu University of Information Technology.

Thirty-third students who have been expelled from school or suspended from school will not be accepted by the school.

Chapter eight? Academic Warning, Trial Reading and Dropout

Article 34 Students whose grades are below 14 credits per semester, or whose required courses are to be retaken exceed 12 credits, will be given a yellow card warning (excluding graduation grades).

Thirty-fifth students who have accumulated two academic yellow cards, or whose compulsory courses have accumulated more than 30 credits but less than 40 credits, will be given academic red cards.

Thirty-sixth students should drop out of school in any of the following circumstances:

(a) during the school study (including suspension) has reached the maximum length of study, and does not meet the conditions for graduation or graduation;

(2) get a red card warning;

(3) Students who have been demoted for trial reading are warned by academic yellow cards again;

(4) The credits required to be re-taught in compulsory courses have accumulated to more than 40 credits;

(five) after the expiration of the suspension period, except for normal reasons such as force majeure, the procedures for resumption of schooling have not been handled, or the application for resumption of schooling has not been applied after review;

(six) suspended from school after being transferred by the school, and the truancy time exceeded one-third of the total school hours;

(seven) did not leave school for two consecutive weeks, and did not participate in the teaching activities stipulated by the school;

(eight) diagnosed by the hospital designated by the school, suffering from illness or unable to continue studying at school due to accidental disability;

(9) Failing to register within the time limit prescribed by the school without justifiable reasons;

(10) I applied to quit school, but my persuasion was ineffective.

Students who should drop out of school due to the provisions of item (2) of this article may apply for demotion and probation, and their school status will not be cancelled temporarily. For specific procedures, please refer to the Administrative Measures for Academic Warning of Undergraduates in Chengdu University of Information Technology.

Article 37 The suspension of students from school shall be decided by the meeting of principals. For students who drop out of school, the school will issue a decision to drop out of school and serve it to me. If it cannot be delivered to me, it will be delivered in the form of a campus announcement. At the same time, it should be reported to the provincial education administrative department where the school is located for the record.

Thirty-eighth students who have objections to the handling of dropping out of school may lodge a complaint according to the Regulations on Handling Students' Complaints in Chengdu University of Information Technology.

Thirty-ninth students who suspend or cancel their school status for other reasons shall go through the formalities of suspension within five working days. Archives and household registration shall be handled in accordance with relevant regulations.

Chapter 9? Graduation and graduation

Fortieth students who have dropped out of school for less than one year may apply for a study certificate, and those who have studied for more than one year may apply for an associate degree certificate.

Forty-first students who have completed all the contents stipulated in the talent training program of this major, but have not met the minimum requirements for graduation credits, and have obtained 85% of the total credits stipulated in the talent training program, may apply for the issuance of a certificate of completion.

Chapter ten? Graduation and degree

Forty-second students graduate, the school should conduct a comprehensive graduation appraisal and audit of graduates. The contents of appraisal and audit include political attitude, ideology, moral quality, study, labor, discipline and health status.

Forty-third students with a student status should complete the contents stipulated in the professional training program and obtain all the required credits within the maximum number of years stipulated in the student status. Those who pass the graduation appraisal and examination and meet the graduation requirements morally, intellectually and physically are allowed to graduate and issued with graduation certificates.

Forty-fourth in the longest academic year, students who have obtained the certificate of completion can obtain corresponding credits through the re-examination of unqualified courses or make-up courses. Those who meet the graduation requirements after examination may apply for renewal of graduation certificate. The graduation date shall be filled in according to the actual date of issuance.

Forty-fifth undergraduate graduates who meet the requirements stipulated in the Detailed Rules for the Granting of Bachelor's Degree in Chengdu University of Information Science and Technology may apply for the award of the corresponding bachelor's degree.

Article 46 The list of graduates shall be put forward by the college, reviewed by the Academic Affairs Office, approved by the principal in charge and issued for implementation. Schools fill in and issue academic certificates and degree certificates in strict accordance with the types of running schools and learning forms determined at the time of enrollment. Academic certificates and degree certificates are produced and issued by the Academic Affairs Office.

Forty-seventh graduates should properly keep the certificate issued by the school, and if it is lost or damaged for some reason, it will not be reissued. If the certificate is lost, the corresponding certificate can be issued according to the relevant provisions of the Ministry of Education after my application and examination by the school. This certificate has the same effect as the original certificate.

Chapter eleven? Data archiving

Forty-eighth materials related to student status are regularly sorted and filed by the Academic Affairs Office, and reviewed and evaluated by the relevant departments of the school and the higher education authorities.

Article 49 The list of students, transcripts of graduates, ID cards of graduates, graduation lists, decisions on awarding bachelor's degrees and other materials within the scope of archives management shall be managed in accordance with the preservation requirements and time limit of the Measures for Archives Management of Chengdu University of Information Technology.

Chapter XII Supplementary Provisions

Article 50 These Provisions shall be implemented by students of Grade 20 15. The original Regulations on the Management of Student Status of Chengdu University of Information Science and Technology (Cheng Xin Yuan Fa [2005] No.27) shall be abolished at the same time.

Article 51 The Academic Affairs Office shall be responsible for the interpretation of these Provisions. If there are major issues, the academic affairs office will discuss them with relevant departments and submit them to the president's meeting for decision.
