First, the examination registration process

According to the provisions of Article 8 of the Requirements for Supervision and Management of Examinations for Non-destructive Inspectors of Civil Nuclear Safety Equipment, we only accept applications for examinations from employees of nuclear safety license holders or license applicants accepted by the National Nuclear Safety Administration.

1. online registration, I (or multiple units) online registration. Select the assessment type and location, fill in the application form correctly and completely, upload electronic photos (small 2-inch red background color bareheaded photos), and the system will automatically conduct preliminary examination.

2. After passing the preliminary examination, the application form that passed the preliminary examination shall be printed in time, signed and sealed by the person in charge of the unit, scanned together with the required attachments, and uploaded for review.

3. After approval, you can choose courses. If you choose the corresponding assessment category in situ, you will immediately enter the payment link; If the off-site assessment class is selected, the applicant will enter the status of pending review after confirmation, and pay for admission after the application point is approved.

4. After selecting a class, the applicant needs to pay the corresponding assessment fee online, and the successful payment means successful entry into the class.

5. Mail the paper application form.

Please refer to the planned assessment date of the corresponding forensics class and update class for the assessment date of the make-up exam.

Second, upload attachment requirements

Upload attachments (except the application form), please copy them on A4 paper, scan and upload them, and the contents should be clear and identifiable. The file format is jpg (pdf format can be used for technical summary reports or papers), and a single jpg file should be less than 2M.

Evidence collection:

1. Print the application form after the network preliminary examination, sign it with the person in charge of the employer, affix the official seal of the employer, and paste a small 2-inch paper color bareheaded photo, then scan and upload it;

2. Copy of the applicant's ID card (double-sided copy on the same page and then scanned and uploaded);

3. A copy of the applicant's highest academic certificate;

4. A valid vision examination certificate issued by a hospital at or above the county level within one year (naked or corrected vision is not less than 5.0, and there is no color blindness or color weakness);

5 copies of relevant qualification certificates already held;

6. To apply for Level 3, a technical summary report is required.

Update:

5. Level II personnel shall submit the inspection report 1 copy prepared by themselves in recent five years' actual work; Level III personnel shall submit research reports or papers on corresponding nondestructive testing methods in recent five years.

Third, matters needing attention in registration:

1. All assessment classes will stop registering 15 days before the assessment;

2. If you are not registering online for the first time, please fill in your ID number and name in the application form and click "Query". Please check the information carefully and correct it in time if it is untrue.

3. Fill in the required contents in the application form correctly. If the ID number or the way and level of application are wrong, you need to void the application form after logging in and fill in the application again. Others can be revised, but the revised application form needs to be re-examined. Please fill in carefully;

4. When filling in the application form, the name of "work unit" must be completely consistent with the official seal, and abbreviations or abbreviations are not allowed. The contact information filled in the application form, such as fax, telephone and email address, is an important way to distribute the notice. Please fill in the form correctly so as not to miss the evaluation.

5. To upload a digital photo, you need to name the file as ID number and name, such as "310123190012345678 Ding Yi", and the digital photo must be a color photo with a red background and no cap;

6. The application form only accepts copies printed directly after the preliminary examination, and does not accept self-filled forms such as hand-filled forms that have not been audited by the network;

7. After online registration, you can check the status of the application form through online login or check the latest status of the application form through the mailbox left in the application form. In order to avoid missing the assessment time, please check the registration status in time and pay attention to the relevant emails at any time;

8. Examination registration adopts the method of online payment of examination fees. UnionPay cards with online payment function should be prepared before registration to avoid unsuccessful class selection due to delayed payment. Successful course selection depends on successful payment.

9. All applications for forensic evaluation are valid in the current year, and will automatically become invalid after the deadline. The original paper application will not be forwarded or returned. If necessary, please make a backup before submission.

Fourth, the reporting conditions

1. In good health, at least one eye in the logarithmic visual acuity chart has naked eye or corrected visual acuity of not less than 5.0, and the color vision is normal.

Among them, those who apply for the second level should have the valid qualification certificate of the first-level personnel with corresponding methods; Those who do not have the valid qualification certificate of the first-level personnel with corresponding inspection methods should have twice the actual experience time. Personnel who have obtained corresponding qualification certificates in other institutions may apply for taking the qualification examination for nondestructive testing personnel of civil nuclear safety equipment at the same level on the premise of meeting the requirements of corresponding academic level and practical experience.

The actual experience time of the third-level personnel is the time after obtaining the second-level qualification certificate of the corresponding method. At least more than half of the experience is engaged in non-destructive inspection activities of civil nuclear safety equipment products or equipment. To apply for level 3, one shall hold two or more valid level 2 qualification certificates, including the nuclear level 2 qualification certificate of the application method; The third-level personnel applying for RT, UT and ET should also have a valid secondary qualification certificate of PT or MT, and the third-level personnel applying for PT, MT, LT and VT should also have a valid secondary qualification certificate of RT, UT or ET.

3. The qualification certificate has not been revoked in the past 3 years.
