Footnote format: author, title (publisher: publishing house, year of publication), page number. Titles should be italicized.

For example, ian Kershaw, The Nazi Dictionary (London: edward arnold, 1985), 84 pages.

Note: don't add a messy p before the page number, just write the page number.
