1. Retirement category: filled in by "normal, special type of work, sick and disabled". 2. Post or type of work: fill in by "cadres, workers and others". Persons who apply for early retirement for special jobs should fill in the name of the job. 3. Professional and technical titles: fill in according to the highest title category at retirement. 4. Worker's technical level: fill in according to the highest technical level at the time of retirement. 5. Work year: fill in according to the time when the insured employees first joined the work. If there is any interruption after taking part in the work, it should be filled in according to state regulations. 6. Insured year: fill in according to the starting time of the first payment by the insured employee unit or individual. 7. Date and year of establishing personal account: fill in according to the time when the insured employee actually established personal account (including supplementary construction). 8.
