Its contents include:

1. If an enterprise changes its name, legal representative, person in charge, office address and registered capital, it needs to go through the formalities for changing its qualification certificate;

2, enterprise qualification certificate replacement, lost replacement;

3, the construction enterprise management personnel work unit changes, apply for safety production assessment certificate work unit change procedures;

4, the construction enterprise management personnel safety assessment certificate replacement, lost replacement;

5. Enterprises go out of the province to undertake business and go through relevant procedures.

1. The enterprise handles the change of enterprise name, legal representative, responsible person, office address, registered capital, etc.

5) Enterprise qualification application form in duplicate;

6) The attachment materials of enterprise qualification declaration are personnel files. If the registered personnel of national practice qualification have not gone through the formalities for changing their work units, they need to provide labor contracts.

1) Application form for renewal of enterprise qualification certificate of Liaoning Provincial Construction Department in duplicate (approved by the Ministry of Construction and provided by the enterprise in triplicate);

2) The Ministry of Construction approved the enterprise to provide a statement that the enterprise qualification certificate published in the national construction industry newspapers or Liaoning Daily and Liaoning Construction Management Information Network was lost and invalid. According to the relevant regulations, the Provincial Department of Construction approved enterprises to provide loss statements published in comprehensive newspapers at or above the provincial level.

3, the construction enterprise management personnel safety production license work unit change

(1) List of changes in work units of safety production assessment certificate for management personnel of construction enterprises of Liaoning Provincial Construction Department (in duplicate, and submitted to Excel spreadsheet at the same time);

(2) An application form for the change of the work unit of the safety production assessment certificate for the management personnel of the construction enterprise of the Construction Department of Liaoning Province (in duplicate);

If the work unit is changed due to the change of enterprise name, the original and photocopy of the enterprise qualification certificate after the change of enterprise name should be provided at the same time, and the audited copy of the application form for the change of the relevant contents of the enterprise qualification certificate of Liaoning Provincial Construction Department, but the application form for the change of the work unit of the safety production assessment certificate of the construction enterprise management personnel of Liaoning Provincial Construction Department is not required.
