Reservation rules

1, some clinics can make an appointment within 14 days in advance, subject to the website display, and do not accept emergency appointments;

2. Neurosurgery accepts appointments for patients over 4 years old.

3, the same department to make an appointment, before the completion of the previous visit, can not make an appointment for the second time;

4. After the appointment is successful, the patient must bring the original ID card when seeing a doctor on the same day. If you don't have an ID card, please bring the original household registration book where the household registration is located;

5. To make an appointment for a follow-up visit, you need to fill in the outpatient magnetic card number or social security card number (social security cards are only available in Shanghai, and you can't make an appointment for Magnolia cards on the website). Patients from other provinces must provide outpatient magnetic card numbers;

6. Huashan Hospital outpatient magnetic card 8 digits, social security card number 9 digits, the first digit starts with English letters, 8 digits (maybe the last digit is X);

7. Cancellation must be made before 16: 00 pm two working days in advance. Cancellation method: log on to the registration platform of 95 169, check the order to be cancelled in the "Reservation Registration List" under "Reservation Management" and click Cancel;

8. If an appointment is made for the next day's clinic, it cannot be cancelled after the appointment is successful. If you make an appointment for outpatient service on the same day or the next day, the number will be taken at 9: 30am-10: 00pm14: 30pm-15: 00pm.
