Procedures and regulations for reissuing student ID cards
1. Students whose student ID cards are lost or damaged should apply to their college for a replacement within 0/5 weeks of this semester and be examined;
2. Each college will send the electronic version of the Statistical Table of Student ID Card Replacement (see attachment 1) to the mailbox of the Student Affairs and Management Section of the Ministry of Education ((xsk @ cqu.edu.cn));
3. Every semester 16 weeks, the staff of the Student Affairs and Management Department will uniformly review the student information. If the student's family location is inconsistent with the basic information of middle school students in the school system and there is no relevant proof, it will not be handled;
4. Within the first17th week of each semester, the staff of the Student Affairs Office will return the list of students who have passed the information review to the college, and the college will stamp the returned paper version of the Statistical Table for Student ID Card Replacement (in duplicate), then collect the student ID card from the Student Affairs Office and reissue the certificate, and then stamp it with the reissued certificate to the secretarial section of the school office;
5. Only in 16 and 17 weeks of each semester, the student ID card will be reissued in colleges and universities within two weeks. In principle, each college only handles it once, and those colleges that have exceeded the processing period or have been reissued will no longer handle it;
6. If the parents of the students move and need to modify the bus interval, the students themselves will hold relevant certificates. After being audited by the Ministry of Education, the staff will modify the original student ID card and affix the official seal, and there is no need to re-apply for the student ID card;
7. Undergraduate students of Huxi Campus apply in the Academic Affairs Office of Huxi Campus.
