Clinical manifestations of tenosynovitis

Tenosynovitis has different symptoms due to different parts, such as stenosing tenosynovitis of styloid process of wrist and radius, tenosynovitis of flexor digitorum tendon, tenosynovitis of flexor digitorum plantarum and so on.

Stenosing tenosynovitis of styloid process of radius appears in the bony process (styloid process of radius) on the thumb side of wrist, showing obvious pain around the bony process, blocked thumb movement and local tenderness. During the self-test, you can hold your thumb tightly in the other four fingers and bend your wrist to the inside of your wrist, which will cause severe pain at the styloid process of the radius.

Tenosynovitis of flexor tendon mostly occurs on the palmar surface of thumb and middle finger, especially when waking up in the morning. The affected finger is characterized by flexion and extension dysfunction, pain sometimes radiates to the wrist, and there is tenderness at the flexion of the finger joint, which can touch the thickened tendon sheath and pea-sized nodules. When bending the affected finger, it suddenly stays in the semi-bending position, and the finger can neither be straightened nor bent, just like being suddenly "stuck". After pulling with the other hand, the finger can move again, which produces the action and snapping sound similar to pulling the bolt, so it is also called "trigger finger" or "snapping finger".

Plantar flexor tenosynovitis is caused by standing and walking in high heels for a long time. The center of gravity of the body moves forward, and the whole body's gravity is concentrated in front of the sole of the foot. Over time, friction, inflammation and exudation between tendon sheath and tendon will induce tendon sheath stenosis or inflammation.

Tenosynovitis that occurs in the wrist of the upper limb often affects the strength of the player, and sometimes there will be movement deformation when hitting the ball. Tenosynovitis, which occurs in the ankle, will feel pain during the exercise and affect the exercise. Tendon sheath is a sheath-like structure that wraps tendon. The outer layer is fibrous tissue, which is attached to bones and adjacent tissues and plays a role in fixing and protecting tendons. The inner layer is synovium, which nourishes tendon and secretes synovial fluid, which is beneficial to tendon sliding. Due to repeated excessive friction, inflammation and edema of tendon and tendon sheath occur, and the wall of fibrous sheath thickens to form a narrow ring. Fibrosis and thickening of tendon make it difficult for tendon to slide in sheath, which is called stenosing tenosynovitis.
