Take a form from the academic affairs office of the college, fill it out according to the requirements on the form, fill in the number and name of the replaced course in front of it, then fill in the number and name of the course you attended, then take the form to the academic affairs office of the starting college (replaced course) and stamp it, and take it back to the academic affairs office of your college for handling in the fourth and nineteenth weeks.
