1. Students apply in writing. Download the Application Form for College Physical Education Health Care Class issued by your school and fill it out one by one according to the requirements of the form.

2. After giving it to the counselor for signature, go to your department for stamp.

3. Bring the original and photocopy of medical records (hospitals at or above the county level) and the application form to your responsible teacher. The teacher will help you choose the relevant health classes.

A copy of the application is kept by the students themselves for future reference.

Registration requirements for health care classes: Students who are not suitable to participate in compulsory physical education class due to illness or physical condition must hold the diagnosis certificate from a hospital above Grade III and Grade B, and be approved by the physical education teacher in their class and reported to the Academic Affairs Office of the Ministry of Sports for the record before they can participate in physical education in health care classes.
