Party A: XXXX College
Party B: Name of the student; College (department) level specialty
Both parties agree to the following terms of the agreement:
1. Party A has enough dormitories for Party B to live in, and Party B's request to stay in school for special reasons is due to Party B's willingness rather than school reasons.
2. According to Article 13 of Chapter II of the Measures for Handling Student Injury Accidents (OrderNo. 12 of the Ministry of Education), the school will not be responsible for accidents that cause personal injury to students in the following circumstances. The identification of accident liability is based on relevant laws and regulations or other relevant provisions:
1. occurs when students go to school, leave school, return to school or leave school by themselves;
2. Students go out on their own or leave school without authorization;
3. After school, holidays, holidays and other school working hours, students stay in school or leave school on their own;
4. Others occur outside the scope of school management responsibilities.
Therefore, Party B shall be punished according to the above four items.
Three. Party B must provide Party A with the following written materials for the record:
1. The student's application form must clearly state:
(1) The student's day accommodation was caused by my voluntary non-school reasons.
(2) The responsibility for personal safety and property loss shall be borne by the students themselves, which has nothing to do with the school.
2. Parents of the students come to the school in person to agree with the students to study for one day, and sign and seal this agreement.
3. The leaders of the college (department) where the students are located sign and affix their official seals.
4. This agreement is made in quadruplicate, one for the student himself, one for the Student Office of the Party Committee of our school, one for the Security Office and one for the student dormitory management center.
5. This agreement shall come into effect as of the date of signature by both parties.
