Treatment results of catastrophic collapse accident of Tuojiang Bridge in Fenghuang County, Hunan Province;

On August 3rd, 2007, at around 6: 45/kloc-0, Tuojiang Bridge in Xidi County, Hunan Province collapsed, resulting in 64 deaths, 4 serious injuries and 18 minor injuries, resulting in a direct economic loss of 39.747 million yuan.

(1) Basic information

Xidi Tuojiang Bridge is an important control project in the construction project of Fengda Highway from Fenghuang County, Hunan Province to Daxing Airport in Tongren, Guizhou Province. The total length of the bridge is 328.45m, the deck width is 1.3m, and the longitudinal slope is 3%. The bridge type is an open-web articulated arch bridge with four holes and a span of 65 m. The pier of this bridge is 33m high and is a multi-arch stone arch bridge. In June 2003, Hunan Provincial Communications Department approved the preliminary design of Fengda Highway Project, and in February of the same year, it approved the commencement report of Fengda Highway Project. Xidi Tuojiang Bridge started on March 12, 2004, and the planned construction period is 16 months. At the time of the accident, the laying of the arch ring and side wall of the bridge and the filling on the arch have been basically completed, and the dismantling work is coming to an end. The construction of the bridge is planned to be completed by the end of August, 2007, and completed and opened to traffic on September 20th, as a gift for the 50th anniversary celebration of Xiangxi Autonomous Prefecture.

The construction unit Fengda Highway Construction Co., Ltd. (hereinafter referred to as "Fengda Company") is a wholly state-owned company under the Xiangxi Autonomous Prefecture People's Government. The design and geological survey unit, Hua Gang Design Institute, is owned by the whole people and belongs to Changsha University of Science and Technology. The Institute has Grade A engineering design certificate, Grade A engineering consulting qualification certificate and Grade A engineering survey certificate in highway industry.

Hunan Luqiao Construction Group Corporation (hereinafter referred to as "Luqiao Company"). Is a wholly state-owned large-scale enterprise, with 28 subsidiaries and shareholding companies (units) under its jurisdiction. It has the qualification certificate of construction enterprise issued by the Ministry of Construction, and obtained the safety production license in July 2006. Luqiao Company implements a three-level management system, and the second-level organization Luqiao No.7 Company is responsible for the specific construction tasks of Xidi Tuojiang Bridge.

Supervision unit: Hunan Jinqu Traffic Consulting and Supervision Co., Ltd. is a limited liability company held by 45 natural person shareholders. With Grade A supervision qualification of highway engineering.

(2) The cause of the accident

1. The direct cause of the accident

Because the masonry materials of the main arch ring of the bridge do not meet the specifications and design requirements, the construction technology of the upper structure of the arch bridge is unreasonable, and the masonry quality of the main arch ring is poor, which reduces the integrity and strength of the arch ring masonry. With the increasing load on the arch, the masonry strength of the main arch ring near No.0 abutment 1 hole is about 3 to 4 meters wide, that is, the lower arch foot section of No.2 abdominal arch reaches the failure limit and collapses, and the whole bridge collapses rapidly due to the multi-arch effect.

2. The main cause of the accident

First, the project management department of Xituojiang Bridge, Fengdagong Dike, Seventh Road Company of Luqiao Company, the construction unit, changed the original main arch ring construction scheme without authorization, and the site management was chaotic, and the main arch ring construction did not meet the specification requirements. When the main arch ring fails to reach the design strength, it begins to take off the frame. Second, the project management of the construction unit Fengda Highway Construction Co., Ltd. (hereinafter referred to as "Fengda Company") is chaotic, failing to seriously urge the construction unit to rectify the construction quality problems found, changing the original design and construction scheme of the main arch ring with the construction unit without the consent of the design unit, blindly reversing the construction period to catch up with the progress, exceeding the authority, and even asking the supervisor not to go to the bridge for inspection. Third, Hunan Jinqu Traffic Consulting and Supervision Co., Ltd., the engineering supervision unit, failed to stop the construction unit from changing the original construction plan of the main arch ring without authorization, and failed to urge the rectification of the construction quality problems found in the main arch ring. When the main arch ring is completed but the strength data is not measured, it has been signed for acceptance. Fourth, Huaying Design Institute, the design and geological survey unit, subcontracted the survey project to individuals in violation of regulations, and the depth of geological survey design was not enough, and the on-site service and design disclosure were not in place. Fifth, the traffic quality supervision departments in Hunan Province and Xiangxi Prefecture have seriously neglected the quality supervision of the bridge project. Sixth, Xiangxi Autonomous Prefecture, Fenghuang County Government and relevant departments of Hunan Province failed to supervise the project approval, bidding, quality and safety production. The state government requested a blind deadline to present a gift to the 50th anniversary of the "State Celebration".

After investigation, it was found that this was a liability accident.

(3) Handling of those responsible for the accident

24 people were dealt with by the judicial organs. These include:

1. Feng Dagong Dituojiang Bridge 1 Arch ring construction team contractor and flaky material supply contractor Xie. Suspected of major engineering safety accidents.

2. He Jie, Minister of Material Purchasing Department of Project Management Department of Road and Bridge Company No.7 Road Company. Suspected of major engineering safety accidents.

3. Wang Zheng, Minister of Engineering Department of Project Management Department of Road and Bridge Company No.7 Road Company. Suspected of major engineering safety accidents.

4. Xia Youjia, Project Manager and Chief of Security Section of Road and Bridge Company No.7 Road Company. Suspected of major engineering safety accidents.

5. Xiao Guoqiang, manager of Road 7 Company of Luqiao Company. Suspected of major engineering safety accidents.

6. Chen Xin, Deputy Chief of Works Section of Highway Bureau of Xiangxi Autonomous Prefecture and Minister of Engineering Department of Fengda Company. Suspected of dereliction of duty

7. Wu Zhihua, deputy general manager and chief engineer of Fengda Highway Construction Co., Ltd., was suspected of abusing his power and accepting bribes.

8. You Xingfu, chief engineer of Xiangxi Autonomous Prefecture Highway Bureau and general manager of Fengda Company. Suspected of dereliction of duty and bribery.

9. Hu Dongsheng, director and party secretary of Xiangxi Autonomous Prefecture Highway Bureau, chairman of Fengda Highway Construction Co., Ltd., and deputy to Xiangxi Autonomous Prefecture People's Congress. Suspected of dereliction of duty and bribery.

10. Yu Yang, deputy director and site supervisor of Fengda Highway of Hunan Jinqu Traffic Consulting Supervision Co., Ltd., was suspected of a major engineering safety accident.

13. Zhang Shicheng, director of Hunan Traffic Construction Quality Supervision Autonomous Prefecture. Suspected of dereliction of duty

33 people were given corresponding disciplinary actions.

These include:

2. Xu Xiaoming, Vice Minister of Safety Production Department of Hunan Road and Bridge Company. Be mainly responsible for the accident. Give administrative demotion and severe warning within the party.

3. Lu, chief engineer of Hunan Road and Bridge Company. Be mainly responsible for the accident. Give administrative dismissal.

4. Liu Xiaodong, director and general manager of Hunan Road and Bridge Company. Be mainly responsible for the accident. Give administrative dismissal and serious warnings within the party.

5. Chen Zhibing, Chairman and Deputy Secretary of the Party Committee of Hunan Road and Bridge Company. Be mainly responsible for the accident. Administrative dismissal, revocation of party posts.

6. Li Hailiang, deputy general manager of Hunan Huada Engineering Co., Ltd., was the deputy director of Changsha Jiaotong University Traffic Design Institute (now Hunan Huagang Traffic Planning and Design Institute). Have important leadership responsibility for the accident. Give serious demerits and warnings within the party.

7. Wu Heping, director of the Institute of Civil Engineering Technology Application of Changsha University of Science and Technology, was the director of the Institute of Traffic Design of Changsha Jiaotong University (now Huagang Traffic Planning and Design Institute). Have important leadership responsibility for the accident. Give serious demerits and warnings within the party.

8. Xu Shawei, Party branch secretary and deputy manager of Jinqu Supervision Company. Have important leadership responsibility for the accident. Give administrative demerits and warnings within the party.

15. Li Deqi, Director and Party Secretary of Hunan Provincial Highway Bureau. Be mainly responsible for the accident. Give dismissal and revocation of duties within the party.

16. Chen Xinfa, Director of Planning Office of Hunan Provincial Communications Department. Be mainly responsible for the accident. Give a demerit.

17. Ang Lee,1993-In March 2006, he served as the director and party secretary of Hunan Provincial Communications Department, and now he is the supervisor of Hunan Provincial Party Committee. Have important leadership responsibility for the accident. Give serious demerits and warnings within the party.

18. Deputy Governor of Xiangxi Autonomous Prefecture, member of the Party Group and director of the State Work Safety Committee. Have important leadership responsibility for the accident. Give serious demerits and warnings within the party.

19. Du Chongyan, Deputy Secretary of the Party Committee and Governor of Xiangxi Autonomous Prefecture. Have important leadership responsibility for the accident. Due to other violations of law and discipline, the Hunan Provincial Commission for Discipline Inspection has filed an investigation and dealt with it together.

(four) the punishment and handling of the relevant units and personnel.

Luqiao Company and Fengda Company were each fined 5 million yuan; Luqiao Company shall dissolve its affiliated Luqiao No.7 Company in accordance with the Company Law and other relevant laws and regulations.

Chen Zhibing, Chairman of Luqiao Company, Liu Xiaodong, General Manager, Lian Fang, Vice Chairman, Hu Dongsheng, Chairman of Fengda Company and You Xingfu, General Manager were each fined 80% of their income in 2006.

Xia Youjia, project manager of Luqiao Company and head of safety department of Luqiao No.7 Company, Xiao Guoqiang, manager of Luqiao No.7 Company, Xiang, deputy director of safety production department and director of project management department of Luqiao Company, Lu, chief engineer of Luqiao Company, Hu Wei, chairman and general manager of Hunan Jinqu Traffic Consulting and Supervision Co., Ltd., Gao Fuliang, deputy manager, Tang Shu, chief engineer and resident supervisor revoked relevant qualifications and post certificates.

Five people, including Xia Youjia, Xiao Guoqiang, Wu Zhihua, You Xingfu, etc., who were investigated for criminal responsibility in connection with the case, and four people, including Lu, Liu Xiaodong, and Tang Shu, who were dismissed for administrative responsibility, shall not be the main person in charge of any production and business operation entity within five years from the date of completion of punishment or punishment.

Under the guidance of the Central Commission for Discipline Inspection, the Ministry of Supervision and the Supreme People's Procuratorate, Hunan Province continues to seriously investigate and deal with the corruption behind this accident.

The Hunan Provincial People's Government was instructed to make an in-depth inspection of the State Council.
