2. Name change application form. I hand-written an application form, wrote down the reasons for changing my name and signed it, and went to the police station to review and seal it. After the approval, I can change my new ID card.
② School:
1. Information change application form. You'd better ask the class teacher first if you want to get it from the Student Affairs Office. )
2. Name change certificate. (The police station prints the certificate, provided that the ID card has been renamed. )
3. The college entrance examination registration information registration form. (There is only one collection. Ask the class teacher or file for it. )
4. Copy of ID card. (Copy of new ID card with changed name)
5. The school submitted materials to the Provincial Department of Education for review and changed its name. (This step will take a long time. Generally, the time for submitting materials in schools is in two periods, namely, the examination and change of freshmen's information and the examination and approval of graduates' application for graduation certificate. Please ask the relevant teachers for the specific time).
