The student ID card and school badge are proof of students' study at school and should be properly kept. With the increasing number of students year by year, the loss of student ID cards is serious. In order to strengthen the management of student ID cards and school badges, according to the Interim Measures for the Management of Student ID Cards and School Badges of Shandong Normal University, the following provisions are formulated:

I. Scope

All college students with formal full-time status can apply for a replacement if their documents are lost.

Second, the procedure.

1. If a student's student ID card is lost or damaged, he/she should apply for a replacement and submit it to his/her college or department for review;

2. Fill in the "Application Form for Shandong Normal University Students to Reissue Student ID Cards"; Fill it out and give it to the counselor.

3. Students who lose their student certificates must report the loss to the newspaper of Shandong Normal University with the "Application Form for Shandong Normal University Students to Reissue Student Certificates" after being examined and sealed by various colleges and departments. This step is handled by the counselor in a unified way, and the loss reporting fee is paid by himself (15 yuan).

4. Students will fill in the Application Form for Shandong Normal University students to reissue their student ID cards (the original certificate of damage must be returned if the certificate is damaged), and pay the fees and handling fees to the hospital and department office; This step is handled by the counselor.

5. The Academic Affairs Department of each college shall be unified to the Academic Affairs Office within the specified time.

Third, time.

Tuesdays in the last week of May and the first week of December every year.

Fourth, charge

There is a handling fee for reissue of student ID card and school badge 10 yuan. Train discount card fee 7 yuan.

Five, please fill in the "Shandong Normal University colleges to reissue student ID cards and train discount cards list", and submit it to the Academic Affairs Office.
