In 2022, the second time (June 21-July 7), an application for accreditation was submitted on the teacher qualification website of China, and the accreditation institution was the junior high school, primary school and kindergarten teacher accreditation applicant of Suzhou Industrial Park Education Bureau.

Those who failed to participate in the first on-site confirmation in 2022 due to special reasons may also participate in this on-site confirmation.

Second, confirm the location on site

One-stop service center of Suzhou Industrial Park (east of the second floor of Industrial and Commercial Building, No.351Suzhou Avenue East, Suzhou Industrial Park).

Third, on-site confirmation time and online reservation requirements

(1) On-site confirmation time: July 9: 00 ~1:30,13: 00 ~16: 00; July159: 00 ~1:30, 13: 00 ~ 15: 30.

(a) the resident identity card within the validity period.

(2) Household registration information or residence permit within the scope of Suzhou Industrial Park (only one of the following is required).

1. If applying at the domicile, provide the applicant's household registration book (including the first page and personal page); If you have a collective account, you need to provide my household registration page in the collective household registration book;

2. If you apply at your place of residence, you need to provide a valid residence permit (the acceptance certificate of residence permit is not recognized). If there is no "industrial park" on the residence card, you need to explain that you actually live in an industrial park. Please bring the paper proof material of "Floating Population Information" issued by the police station where I actually live in advance;

3. Fresh graduates, undergraduates and graduate students studying full-time in local universities (including branch schools and research institutes) are required to provide valid original student ID cards. If the fresh graduates fail to pass the online student status verification, they need to provide the Online Student Status Verification Report of the Ministry of Education or the Employment Recommendation Form for Fresh Graduates;

4. Servicemen and armed police shall provide military or police officers' certificates. If the service place cannot be shown on the certificate, it is also necessary to provide the personnel relationship certificate issued by the subordinate unit or the personnel department of the unit. The format of the certificate depends on the provisions of the unit or unit, and the certificate should clearly indicate the applicant's service location;

5. Hong Kong, Macao and Taiwan residents who apply for recognition in their place of residence shall provide residence permits issued by local public security organs; Hong Kong, Macao and Taiwan residents who apply for recognition in the location of the primary and secondary school teacher qualification examination shall provide a residence permit for Hong Kong, Macao and Taiwan valid for 5 years or a pass for Hong Kong and Macao residents to and from the mainland and mainland travel permit for taiwan residents.

(3) education. If the academic information is successfully compared by the online newspaper system, it does not need to be submitted. Academic qualifications obtained in Hong Kong, Macao and Taiwan and abroad should also provide the corresponding academic degree certification issued by the Study Abroad Service Center of the Ministry of Education.

Special note: In the process of material review, for the academic qualifications that cannot be directly compared and verified by the online newspaper system (except the academic qualifications of secondary vocational schools), the applicant should submit the electronic registration record form of the academic certificate of the Ministry of Education or the certification report of China higher education (online application in www.chsi.com.cn, Xuexin.com), otherwise it will be regarded as unqualified academic qualifications and will not be accepted. Applicants are advised to verify their academic qualifications in Xue Xin. Com, and apply for the certification report as soon as possible if it cannot be verified; Applicants with Hong Kong, Macao and Taiwan education or foreign education should be certified in the online service hall () of the Study Abroad Service Center of the Ministry of Education in advance, so as not to affect the appraisal.

(4) Putonghua proficiency test grade certificate (if it can be verified by the online newspaper system, it is not necessary to provide it).

(five) "primary and secondary school teacher qualification examination certificate" and "teacher professional ability certificate" through the online newspaper system verification, do not need to provide.

20/kloc-full-time normal graduates from ordinary colleges and universities who have enrolled in or before 0/4 years apply for direct recognition of teacher qualifications. If the word "normal" is not clearly marked in the graduation certificate, it is necessary to provide personal study transcripts (qualified in compulsory subjects such as pedagogy and educational psychology) and one of the following materials:

1. A copy of the professional enrollment plan document issued by the higher education department of the graduation school at the time of enrollment, indicating that the major is a full-time normal major (stamped with the official seal of the educational affairs, archives or development planning department of the graduation school);

2. A copy of the enrollment list of full-time normal students in the current year indicating the applicant's name and major (marked with the normal major and stamped with the official seal of the graduate school enrollment or archives department);

3. The professional column of the report card in the personal study file is marked with the word "Normal" (the copy is stamped with the official seal of the school educational administration department or the personnel file management department);

4. The "Recommendation Form for Employment of Fresh Graduates" is marked as an ordinary graduate (a copy is stamped with the official seal of the personnel file management department);

5. The professional column of the admission notice issued by colleges and universities is marked with the word "normal" (the copy is stamped with the official seal of the graduate school enrollment or student status management department);

6. 1999 Graduates majoring in normal education who entered the university before the enrollment expansion, although there is no word "normal" on the graduation report card, have systematically studied pedagogy and educational psychology and passed the educational practice (the copy is stamped with the official seal of the school educational affairs or personnel file management department).

(six) Hong Kong, Macao and Taiwan residents are required to submit a certificate of no criminal record issued by the relevant departments of Hong Kong Special Administrative Region, Macao Special Administrative Region and Taiwan Province Province. If necessary, Hong Kong and Macao applicants can apply for a letter issued by our province through an appraisal institution, and ask the relevant departments to issue a certificate of no criminal record.

(7) Physical Examination Form of Applicants for Teacher Qualification in Jiangsu Province. Applicants for teacher qualification shall go to the designated hospital for physical examination at the specified time. In the first half of 2022, new teachers who have participated in the physical examination organized by Suzhou Education Administrative Department can replace the physical examination report with the physical examination certificate issued by Suzhou Education Administrative Department.

(eight) recent 1 inch bareheaded color photos on a white background. A photo of a formal paper certificate used in applying for teacher qualification certificate, with the size of 25mmX35mm, should be in the same format as uploaded by the online newspaper system, with the name and ID number indicated on the back of the photo and pasted on the photo pasting page (attachment 1).

(nine) "teacher qualification on-site confirmation material bag cover" (Annex 2), do not need to paste on the material bag.

All the above materials are originals without copies. If the materials submitted by the applicant are incomplete or do not meet the requirements, they shall be completed before the end of the acceptance period.

Verb (short for verb) others

(1) The applicant is requested to make on-site confirmation according to the time, place and requirements specified by the recognized institution. If the application is not completed within the specified time due to reasons such as missing the application time, choosing the wrong accreditation body or on-site confirmation point, incorrect application information or incomplete submission materials, the accreditation body will not accept the application, and the responsibility will be borne by the applicant.

(2) Please ask the applicant (or client) to strictly abide by the epidemic prevention and control regulations, wear a mask correctly when entering the hall, scan the registration code of key places, consciously accept the temperature test and cooperate with the epidemic prevention inspection. (If there is any change in epidemic prevention requirements, the site requirements shall prevail. Please consciously keep a safe distance of "one meter long" when handling business, try to reduce the stay time, that is, leave immediately to prevent gathering. (3) If the applicant cannot go to the on-site confirmation point in person for personal reasons and needs to entrust others to handle it, please fill in the Personal Power of Attorney for On-site Confirmation of Teacher Qualification (Annex 3) and submit the materials as required.

(4) Applicants who have obtained the corresponding graduation certificate can handle the mailing business of teacher qualification certificate, and need to fill in the Application Form and Book for Mailing Teacher Qualification Certificate (Annex 4) and show the original academic certificate on the spot.
