1. Personal written application (specify certificate name, professional level, issuing date and explain the reasons for reissue in detail).
2. Submit a copy of the "Registration Form for Qualified Persons" that has been filed, and affix the official seal of the unit.

If the certificate is lost, the applicant shall provide a full copy of the original newspaper (the newspaper must be above the municipal level and have a national unified newspaper number) which should reflect the name of the holder, the name of the certificate held, the qualification grade, the time of obtaining the certificate and the certificate number; If the certificate is replaced, the applicant shall provide the damaged original certificate.

5. My recent bareheaded 2-inch photo.
